Budzisz się i nie czujesz zapachu porannej kawy. Dla wielu z nas to tylko irytująca pamiątka po przebytej infekcji wirusowej, z którą po prostu trzeba nauczyć się żyć. Eksperci z czołowych ośrodków badawczych biją jednak na alarm.

Nagłe pogorszenie tego zmysłu może być wczesnym zwiastunem poważnych problemów neurologicznych, wyprzedzającym inne, widoczne objawy o całe lata.

Zmysł, który latami ignorowaliśmy

Przez dekady węch był traktowany przez medycynę po macoszemu, a lekarze nierzadko odsyłali pacjentów z kwitkiem. Wszystko zmieniła pandemia COVID-19, która wywołała bezprecedensowe zainteresowanie naukowców sposobem, w jaki nos komunikuje się z ludzkim mózgiem. W badaniach opublikowanych w 2023 roku na łamach „Laryngoscope” ujawniono, że aż 60 procent zakażonych wirusem pacjentów doświadczyło tymczasowych lub długotrwałych zaburzeń węchu.

Problem jest jednak znacznie szerszy. Szacuje się, że nawet 22 procent populacji boryka się z różnymi formami upośledzenia tego zmysłu – od całkowitej utraty, po fałszywe, skrajnie nieprzyjemne rozpoznawanie zapachów. Co ciekawe, ludzkie opuszki węchowe są jednym z niewielu obszarów w dorosłym mózgu, które potrafią tworzyć nowe neurony. Stanowią one jednak niezwykle wrażliwą bramę, przez którą do układu nerwowego wnikają wirusy, bakterie i toksyny.

Mroczne powiązania z Parkinsonem i Alzheimerem

Utrata węchu może być czymś znacznie poważniejszym niż tylko skutkiem ubocznym alergii czy przewlekłej infekcji zatok. Jak zauważa dr Zara M. Patel, dyrektorka programu Stanford Initiative to Cure Smell and Taste Loss, brak tego zmysłu bywa jednym z najwcześniejszych zwiastunów choroby Parkinsona, Alzheimera czy otępienia z ciałami Lewy’ego. Zmiany węchowe obserwuje się również u osób cierpiących na depresję czy schizofrenię.

Nadzieja w walce z chorobą Alzheimera. Kontrowersyjna terapia genowa „przeprogramowuje” komórki mózgowe

W przypadku choroby Parkinsona proces obumierania komórek produkujących dopaminę rozpoczyna się na długo przed wystąpieniem drżenia rąk czy problemów z poruszaniem się. Neurolodzy z Uniwersytetu Kalifornijskiego w San Francisco (UCSF) podejrzewają, że toksyczne białka uszkadzające ludzki mózg mogą w pierwszej kolejności gromadzić się właśnie w opuszkach węchowych, dając o sobie znać poprzez anosmię (utratę węchu).

Fizjoterapia dla nosa, czyli jak odzyskać zapach

Czy uszkodzony węch można naprawić? Okazuje się, że medycyna nie jest tu całkowicie bezradna. Naukowcy z Uniwersytetu Pensylwanii (twórcy standardowego testu powonienia) oraz eksperci ze Stanfordu wskazują na skuteczność treningu węchowego, który w swoich założeniach przypomina klasyczną fizjoterapię. Ćwiczenia polegają na świadomym, dwukrotnym w ciągu dnia wąchaniu czterech wyrazistych aromatów – najczęściej cytryny, róży, goździków i eukaliptusa.

Dzięki neuroplastyczności mózgu, powtarzalność tych działań zmusza układ nerwowy do generowania nowych komórek receptorowych. Wyniki badań klinicznych potwierdzają, że u około 30 procent pacjentów następuje mierzalna poprawa po 12-16 tygodniach stymulacji zapachowej. Amerykańscy badacze odnotowali co prawda, że zastosowanie sterydowych płukanek zatok może podnieść tę skuteczność do 50 procent, jednak trzeba to wyraźnie zaznaczyć: tego typu silna kuracja farmakologiczna nie jest domowym sposobem na katar i bezwzględnie wymaga ścisłej konsultacji z lekarzem specjalstą.
