Reading these prompted me to pack out Porter/Teisberg's "Redefining #HealthCare" again.
It remains an interesting read, but it is very influenced by the time of writing and geography.
Any suggestions for sources with similar depth & breadth, but that

Nononono that's not how the #immuneSystem works! I think your very rational point regarding "?what must be set to zero for this to make sense to a systems-architect" is a most excellent one. Any degree of immunity in #healthcare workers contributes to transmissible pathogens being, well, less-transmitted. When these folks are spending all their time caring for the sick and infirm, that's a lot of moments of reduced-transmission (into a ripe breeding-ground for the pathogen) that happen as a result. Don't even get me started on what happens when a whole #hospital ward changes status from some sort of "normal" status to some sort of "infectious" status. There are a LOT of things that have to be valued at (or near) zero for this to make sense.Also keep in mind that exposure to the pathogen or the vaccine carries some risk... These folks are willing to take that risk - essentially - for free! What a treat.
But wait there's more! These healthcare workers are governed by a famously unsympathetic regulator, who will have little sympathy should standards have fallen because n of the workers were off sick. Those who do go into work are willing to carry that risk, also for free! And even more, some of them might be feeling a bit rubbish too, making it even less probable they can meet those regulatory standards. And they're willing to carry that risk for free too!
I think there's a heap of stuff being valued at zero here, and this is just that which is apparent in a very superficial rant...

Adult outcomes by parental, school and postcode aggregated income in childhood—A descriptive analysis of the cohorts 1981–1989 in Finland
https://journals.plos.org/plosone/article?id=10.1371/journal.pone.0327364Finland has a low economic inequality and one of the most desegregated educational systems in the OECD countries.
"On average across the cohorts, compared to the highest income groups, the lowest tenth of parental, school and postcode income had, respectively, a 1.8, 1.7 and 1.4-fold risk of death, 3, 1.3 and 0.9-fold risk of low education, 2.5, 1.8, and 1.4-fold risk of long-term unemployment. People with a lower childhood parental or school income were more likely to live with a partner and have children than people from higher income backgrounds. The differences by school and postcode income in the education outcome were small. We observe increasing differences in the education and employment outcomes by parental and, to a lesser extent, school income."

Population-health impact of new drugs can be net-negative.
During 2000–20, NHS coverage of new drugs displaced more population health than it generated. Our results highlight the inherent trade-offs between individuals who directly benefit from new drugs and those who forgo health due to the reallocation of resources towards new drugs.
