  1. As a GP in Germany, I struggle with the claim that “people don’t want to work.” In daily practice, I see the opposite: many patients push themselves to keep working, even when they should rest. They worry about burdening colleagues, unfinished tasks, or upcoming deadlines.

If we want to understand rising sick leave, we need to look at real factors: increasing workload, constant pressure, lack of recovery time, social isolation, and overall exhaustion. Reducing this to a question of “motivation” ignores both evidence and lived experience.

In my experience, people who genuinely try to avoid work are a tiny minority. Framing the issue this way feels similar to old narratives about unemployment—oversimplified, misleading, and disconnected from reality.

Reading these prompted me to pack out Porter/Teisberg's "Redefining #HealthCare" again.

It remains an interesting read, but it is very influenced by the time of writing and geography.

Any suggestions for sources with similar depth & breadth, but that
    - are more recent?
    - are more EU/UK focused?
    - have a stronger focus on primary #MentalHealth care?

  5. A comment discusses perspectives on the move from a "relationship-oriented and community-oriented approach to a market-oriented and profit-oriented enterprise" in #PrimaryCare:

It points to this interesting interview study describing (and for once explicating) 12 concrete market mechanisms enacting three market principles:

  6. Proud to have published my first paper - on informed consent in primary care e-consultation systems! tl;dr existing system designs consent patients poorly, but better user interface design might deliver better-informed consent #GP #PrimaryCare #research humanfactors.jmir.org/2026/1/e

  7. Learning primary care in a hospital is equivalent to learning forestry in a lumber yard.
