  1. Oh fuck, I wanted to send this out earlier, but better now than too late: If you are #trans in the #Netherlands and still on long waiting lists to start the medical track:

    Tomorrow morning
    Psytrans will very likely open their signup (they didn’t the last two months, and the trend is that they do every third month) at 09:00:00 (SHARP!!) which will be done in a first-come-first-serve manner.

    What is new is that they are now contracted
    with a bunch of health insurance companies, including Menzis, CZ and a bunch of smaller ones. Even if yours isn’t on the list, if you sign up now, your intake might be in January in the new year, giving you the opportunity to change beforehand.

    In my case it took them about a month to give me my first diagnosis, then about 1.5 months to get a single appointment with an external psych for the second opinion, and then a very unreasonable amount of time to get an Endo, but I think I also got unlucky, because they first tried to get a contract with someone in Eindhoven which failed after a while and delayed my referal to Rotterdam. In Rotterdam they did however accept my DIY-HRT since the time for which I had blood-tests as counting for the 1-year-of-HRT requirement for a referral to bottom surgery, and Psytrans as an org was also quite supportive of it and even tried to talk my back-then GP into prescribing me blood tests.

    This might very well be your best chance to get treatment fast for possibly another three months! Don’t miss out on it, if this is relevant for you and don’t assume that being a minute late will still be fine, assume that it’s gone right away! (But still check if you notice that you are late, you might still have luck!)

    I just saw a few things:

    ∘ Psytrans, my Dutch Genderclinic is now contracted with four health-insrance providers: Menzis (mine), OWM Zorgverzekeraa Zorg en Zekerheid, CZ / Ohra / Nationale Nederlanden, and ONVZ. That means you
    no longer need a (non-existing) restitutie-policy anymore to get full coverage from your health-insurance!

    ∘ Today was one of the days where they announced whether they have space and they did not have any. This sounds bad, but it means that there is a
    pretty good chance that they will have space next month! (Previous period was also three months between opening the list.)

    ⇒ If you have Genderdysphoria in the Netherlands and need an official diagnosis for access to care, you have a real chance to get a pretty reasonable offer, if you sign up on
    their website, on the 6th of october at 09:00:00 (sharp!!!), since it’s first-come-first-serve to get on their rather short 3 month waiting list!

    (Seriously though: when I say “sharp”, I mean it. This is competitive and pretty much the only official place in NL with this kind of deal, do not be a minute late or there is a very real chance that it will be too late!)

    Tomorrow is the first Monday of the month. That means that
    #Psytrans will announce whether they have space on their waiting list for their gender-clinic.

    If you have some money and a good health insurance and are in the Netherlands, this is probably the fastest way to get official treatment here!

    Be there at 09:00:00 sharp!
    If they have spaces, it’s first-come-first-serve for a short waiting list that actually complies with Dutch law.

    The last time they opened the waiting list was in April, I’ve heard (take it as a fourth-hand rumor) that on average they open it like every second months, which would mean that there is a good chance this time around!

  4. I just had a consultation about my surgeries with #Psytrans.

    There are some really good news (
    #transjoy) and some instances of #DutchHellCare:

    Let’s start with the good stuff:

    My endocrinologist actually did a nice thing and accepted my detailed records about how I did
    #OpenHRT combined with the one bloodtest I payed myself as sufficient evidence to accept my actual starting date as starting date for HRT.

    This means that the Dutch healthcare system now accepts that I’ve actually completed my first year of HRT, which is the requirement to start the process for
    #BottomSurgery. 😊

    In that regard I’m pretty clear on what I want and at the same time very open about the details: Full
    #vaginoplasty, no half-meassures like pure #orchiectomy or zero-depth. How exactly they are doing it is something that I don’t particularly care about, which is good, because this is also pretty much the extend to which they let you choose.

    Accordingly this part of the conversation was over pretty quickly and we moved to
    #FacialFeminisationSurgery…

    And the first issue is that it is relatively hard to get it covered. You essentially need to have issues passing, because otherwise it would be considered a cosmetic surgery which isn’t covered. Annoying, but on the other hand also somewhat understandable. The more annoying thing is that hair-transplants are almost never covered anyways,

    Which brings us to cost…

    So first of all Dutch hell-insurance only pays up to Dutch rates which are not cost-covering for bottom surgery (in case anyone was wondering why the waiting-lists are long: This is one of the reasons, hospitals loose money on it), so of the 20.000€ that one could expect to pay in Thailand, Dutch insurances would only pay 14.000€. So a 6000€ difference probably + travel cost. A lot of money, but I could cover that without too many issues.

    Now full FFS can apparently get to 40.000€…

    Which would not be completely impossible, but in “pillage retirement-savings”-territory…

    Well, in the end I decided to go onto Radbout’s waiting list for a combined surgery (which would also be fully covered by health-insurance, which would be a huge benefit), so if I manage to hit the weight-targets, I might get surgery in maybe like two years or so?

    Speaking of it: I really need to loose weight, that will definitely be the biggest problem going forward…

  5. On my way to the maasstad ziekenhus in Rotterdam to hopefully finally receive an official prescription of #HRT!

    I still have a multi-year supply of E and significant amounts of Cypro at home/on the way to me, but ideally I won’t need them anymore. Still, I will NOT get rid of them, I trust the healthcare systems of this and other countries WAY too little.

    That said, If everything goes well my dose earlier today may have been the last time I engaged in #OpenHRT. 😊

    Since I started on the fourth of February at noon, this also means that I started exactly 8 months early, 2 days after receiving my first official diagnosis of #genderdysphoria and about 7 months after I got my second one. #Psytrans, what the fuck was that pointless delay about?

    The most interesting question will now be whether they believe me about my starting dates (I can give exact data) and will actually treat me as being the better part of a year on HRT…

  6. Just had another talk with a psychologist from #psytrans, my gender-clinic. Mostly it was discussing general stuff about #OpenHRT, but she made one point in favor of getting a private bloodtest that is definitely worth considering: If I can show a bloodtest, that will show the Endo a definite starting point and they will treat me as if that was the point I started. Otherwise the start will from their perspective be when I start with them…

  7. So #Psytrans now told me that they cannot do anything about the waiting times and that they furthermore cannot get me prescriptions for blood-tests either and that if my GP (the kankerkuts I’ve been ranting about) won’t help me, I’m on my own…

    Can’t someone in this godforsaken country hire a bunch of endocrinologists, I’m sure there will be plenty of candidates if you announce that it is specifically for trans people and that you are not going to check the credentials too closely, if the applicant is trans‽

    It’s not like a significant number of them has a lot less of a clue than some of the more informed people in the DIY-community!

  8. Back from my GP to request a blood test…

    I told them about doing #OpenHRT, that there is no way in hell that I’ll ever stop, that #GenderGP didn’t work out, and that #Psytrans told me to get bimonthly blood-test, that I have a gut-feeling that my E is still low but that I would prefer to adjust my dose based on measurements and not gut-feelings.

    You would think that that would be enough to prescribe a simple #BloodTest, but instead there was a lot of concern-trolling and I was told that they’ll call me back later today after they discussed it amongst colleagues.

    Let’s see…

    And I’m serious: If they don’t give me a blood test, I will increase the dose even if it is only to spite them. Fuck all Gatekeepers!

  9. When I’m complaining about the official #transition ing process, let’s continue a bit. The only reason why I already have my diagnosis is that I have the financial means to go for private healthcare. To be fair, in the Netherlands this isn’t actually all that expensive if you pick the right #HealthInsurance company (Read: Menzis or one or two others) that will fully cover all legal costs even if the provider hasn’t contracted with them.

    Now #Psytrans that I am with (the only gender-clinic without insane waiting times) has no contracts with any health-care providers and I picked my health-insurance accordingly. Still: I have to pay first and only get to reimburse the cost later.

    So here is the invoice that I paid yesterday, it does not include the cost for the intake because that was still in 2023.

    Now I am in the very luck situation where I have the liquidity to pay that on short notice without any problems. Many people do not!

    The takeaway is that the system is not just #transphobic but also #classist.
