@[email protected] @[email protected] At least here in the Netherlands they don’t even pay for dilators, while insisting that they are super necessary… 🙄

CW: trans healthcare in NL (abstract, bot + and -)
I just noticed something (actually ran into it when I was looking for something completely different): Radboud MC (The university hospital in Nijmegen) is confident enough in the quality of their transgender healthcare to put their team on their website! As I’ve said before, the fact that the fuckers in Amsterdam are too ashamed of the shit quality of care they offer to do that, kinda already tells you what you need to know about them…
So far I’m not super impressed with Radboud, but I also haven’t been pushed to hating them, and this is one of the signs that indicate to me that they might actually be decent!

CW: further waiting lists for bottom surgery
So, back on the train to Den Bosch.
A few more details: They were willing to believe my height and weight and accepted that (😊), we were talking procedures and that I want a full vagionplasty and the other further stuff.
Essentially I’m now on three further waiting lists:
1. I have to have a conversation with their own psych (:blobcat_eyeroll:) to make sure that I really know that I want it I guess, which might happen in June or so. Supposedly that is just one appointment that can be a call.
2. Because I also want facial surgery there will be a separate intake later for that, but it shouldn’t affect the timeline afterwards.
3. Once the psych has given the okay, they’ll put me on the actual, actual, definitely for real this time, waiting list for the actual surgery. That is the waiting list that was until recently 14 months, but is seemingly shrinking significantly at this point, currently at a year and they expect it to go down further but can’t tell yet.
In any case, it will at least be more than a year until the actual surgery, so… :blobcat_eyeroll:

I’m now in the mood where I want to just take a high dose of E or something…
Apparently you can get SRS in Thailand for under 20.000€… It’s a lot of money, but if it saves me two years waiting, it might be worth it…
Still, Fuck this country!

Oh fuck, I wanted to send this out earlier, but better now than too late: If you are #trans in the #Netherlands and still on long waiting lists to start the medical track:
Tomorrow morning Psytrans will very likely open their signup (they didn’t the last two months, and the trend is that they do every third month) at 09:00:00 (SHARP!!) which will be done in a first-come-first-serve manner.
What is new is that they are now contracted with a bunch of health insurance companies, including Menzis, CZ and a bunch of smaller ones. Even if yours isn’t on the list, if you sign up now, your intake might be in January in the new year, giving you the opportunity to change beforehand.
In my case it took them about a month to give me my first diagnosis, then about 1.5 months to get a single appointment with an external psych for the second opinion, and then a very unreasonable amount of time to get an Endo, but I think I also got unlucky, because they first tried to get a contract with someone in Eindhoven which failed after a while and delayed my referal to Rotterdam. In Rotterdam they did however accept my DIY-HRT since the time for which I had blood-tests as counting for the 1-year-of-HRT requirement for a referral to bottom surgery, and Psytrans as an org was also quite supportive of it and even tried to talk my back-then GP into prescribing me blood tests.
This might very well be your best chance to get treatment fast for possibly another three months! Don’t miss out on it, if this is relevant for you and don’t assume that being a minute late will still be fine, assume that it’s gone right away! (But still check if you notice that you are late, you might still have luck!)

I just saw a few things:
∘ Psytrans, my Dutch Genderclinic is now contracted with four health-insrance providers: Menzis (mine), OWM Zorgverzekeraa Zorg en Zekerheid, CZ / Ohra / Nationale Nederlanden, and ONVZ. That means you no longer need a (non-existing) restitutie-policy anymore to get full coverage from your health-insurance!
∘ Today was one of the days where they announced whether they have space and they did not have any. This sounds bad, but it means that there is a pretty good chance that they will have space next month! (Previous period was also three months between opening the list.)
⇒ If you have Genderdysphoria in the Netherlands and need an official diagnosis for access to care, you have a real chance to get a pretty reasonable offer, if you sign up on their website, on the 6th of october at 09:00:00 (sharp!!!), since it’s first-come-first-serve to get on their rather short 3 month waiting list!
(Seriously though: when I say “sharp”, I mean it. This is competitive and pretty much the only official place in NL with this kind of deal, do not be a minute late or there is a very real chance that it will be too late!)

Tomorrow is the first Monday of the month. That means that #Psytrans will announce whether they have space on their waiting list for their gender-clinic.
If you have some money and a good health insurance and are in the Netherlands, this is probably the fastest way to get official treatment here!
Be there at 09:00:00 sharp! If they have spaces, it’s first-come-first-serve for a short waiting list that actually complies with Dutch law.
The last time they opened the waiting list was in April, I’ve heard (take it as a fourth-hand rumor) that on average they open it like every second months, which would mean that there is a good chance this time around!

I’m about to write an email to the corresponding author of the #Progesterone study. This is my current draft:
Dear Koen,
Thoughts anyone?
in 2024 you and your team registered a study with BMC Pharmacology and Toxicology in which you declare that you will perform a 1 year study on the effects of progesterone and higher Estradiol-levels in the treatment of transgender individuals. Considering that this was now 1.5 years ago, that you had essentially finished recruiting of participants at the time, and that this is a fully pre-registered study, I am somewhat surprised that you still seem to not have published the results.
I am sure that you are aware that many transgender individuals are awaiting these results not least because many endocrinologists even outside VUMC are refusing to prescribe progesterone, pointing to this supposedly soon to be available study. Naturally this is creating avoidable tension and I am aware of several trans women who have expressed the intention of self-medicating progesterone on top of their officially provided HRT regiments.
It is with this background, that I’d like to know what the current status is and when we can expect the final publication.
Best Regards,
Dr. Fiona Weber
I figure that this is one of those places where dropping my PhD might be useful; I’m a bit uncertain about the ambiguity regarding my own situation: It’s obvious from my website (trivial to find from my email) that I’m a transgender cryptographer, not a medical doctor, but it’s not like I claim to be. It might come across as me implying it in a deceiving manner though, which could be harmful… So I’m not sure if I should just say it directly to avoid that scenario.

in 2024 you and your team registered a study with BMC Pharmacology and Toxicology in which you declare that you will perform a 1 year study on the effects of progesterone and higher Estradiol-levels in the treatment of transgender individuals.

I’m about to write an email to the corresponding author of the #Progesterone study. This is my current draft:

Just got a call from the fuckers in Amsterdam (VUMC) whether they need to keep me on their waiting list. I told them that I’m essentially through the entire process (no thanks to them), and should now be put on Radboud’s surgery waiting-list any day now and that they can remove me from theirs, since I prefer to work with psytrans. (Maybe I should have said “because unlike for you, I do have some trust into those people…”)
I did ask them where in the waiting list they are currently and was told that they are now dealing with people who were referred to them in 2020…
I also asked about the #progesterone study that they should really have published already, but the person who called me claimed to not really know anything about, go figure…

So #DutchHellcare apparently refuses to even put me onto the fucking waiting lists for surgeries until I have a BMI ≤ 30. 😞
I guess one of the issues with that is that the reward for getting there is to be put on a waiting list for an event at which you are going to be put on another waiting list.
That said: It definitely provides motivation, so let’s see how things are going to go…

I just had a consultation about my surgeries with #Psytrans.
There are some really good news (#transjoy) and some instances of #DutchHellCare:
Let’s start with the good stuff:
My endocrinologist actually did a nice thing and accepted my detailed records about how I did #OpenHRT combined with the one bloodtest I payed myself as sufficient evidence to accept my actual starting date as starting date for HRT.
Thank you Gina, I really and actually appreciate that! (#NameAndPraise)
This means that the Dutch healthcare system now accepts that I’ve actually completed my first year of HRT, which is the requirement to start the process for #BottomSurgery. 😊
In that regard I’m pretty clear on what I want and at the same time very open about the details: Full #vaginoplasty, no half-meassures like pure #orchiectomy or zero-depth. How exactly they are doing it is something that I don’t particularly care about, which is good, because this is also pretty much the extend to which they let you choose.
Accordingly this part of the conversation was over pretty quickly and we moved to #FacialFeminisationSurgery…
And the first issue is that it is relatively hard to get it covered. You essentially need to have issues passing, because otherwise it would be considered a cosmetic surgery which isn’t covered. Annoying, but on the other hand also somewhat understandable. The more annoying thing is that hair-transplants are almost never covered anyways,
Which brings us to cost…
So first of all Dutch hell-insurance only pays up to Dutch rates which are not cost-covering for bottom surgery (in case anyone was wondering why the waiting-lists are long: This is one of the reasons, hospitals loose money on it), so of the 20.000€ that one could expect to pay in Thailand, Dutch insurances would only pay 14.000€. So a 6000€ difference probably + travel cost. A lot of money, but I could cover that without too many issues.
Now full FFS can apparently get to 40.000€…
Which would not be completely impossible, but in “pillage retirement-savings”-territory…
Well, in the end I decided to go onto Radbout’s waiting list for a combined surgery (which would also be fully covered by health-insurance, which would be a huge benefit), so if I manage to hit the weight-targets, I might get surgery in maybe like two years or so?
Speaking of it: I really need to loose weight, that will definitely be the biggest problem going forward…
