  4. Adults who reported having COVID-19 at least twice were significantly more likely to be diagnosed as having a new medical condition than those who reported never having been infected, according to a study published this week in Canada Communicable Disease Report. 

    It’s well known that preexisting conditions can increase the risk of severe COVID. The new research suggests that the relationship between COVID and chronic illness may run in both directions.

  5. Drove my friend who recently had rotator cuff surgery to her physical therapy appointment at Stanford Medicine Outpatient Center. Sign on the entrance door says "Please wear a mask."

    How many people were masked? Me, in my N95. The physical therapist, in a basic blue mask. No one else that I saw.

  13. Manipulation of information in times of crisis: evidence from Covid excess mortality

    “Governmental information manipulation has been hard to measure and study systematically. This paper provides comparable country-level estimates of the extent of misinformation in official data, in a large and diverse set of countries. We hand-collect data from a variety of sources in 134 countries to estimate misreporting of Covid mortality in 2020-2021. We find that information manipulation by governments during this period was remarkably widespread: between 45%–55% of governments misreport the number of deaths. The lion’s share of misreporting cannot be attributed to a country’s capacity to accurately diagnose and report deaths. Contrary to some theoretical expectations, information manipulation takes the form of downplaying rather than exaggerating the severity of the crisis.

    What characterizes governments that manipulate information? We examine the role of macroeconomic incentives, culture, audience sophistication, and institutions. The evidence strongly suggests that misreporting is higher where governments face fewer social and institutional constraints; in countries holding elections during this period; and in countries with a communist legacy.”

I'm seeing a quite a few investigative journalism articles (from usually decent, well researched publications) writing about medical issues, past-tensing COVID, and then going on to suggest a variety of reasons why there's a sharp rise in such-and-such a rare medical condition, illnesses, deaths, year around crowded hospitals etc.

    So, at no point in your investigation did you, or your editors "uncover" that the pandemic never ended. Or that #COVID is a still a really really nasty novel virus that fucks you up in so many ways (often without you knowing for a while) and getting it two or three times a year might just help you explain a few of these things?

  31. Macitentan - an approved dual endothelin receptor antagonist used for pulmonary hypertension. In hamsters it reduced iron accumulation, cellular senescence, and bone loss. Two windows were tested -acute (days 1-3) and delayed (16-30).
    The human piece. Plasma ET-1 in 4 vs 4 people, ex vivo lung, 1 sample vs 1 cartilage, 3 controls vs 5 post-COVID, cross-sectional, pre-pandemic controls. The authors - the cohort is powered only to detect very large effects and call the finding exploratory. Causality and chronicity aren't established.

    The damaged growth plate is a finding in juvenile animals, so relevant to children, whose growth plates are still open. The adult human finding sits elsewhere - articular and subchondral. Bone after COVID in a child and in an adult aren't the same thing.

    The authors themselves add a recommendation here. Given the growth plate's role in longitudinal bone growth, and a growing but still scattered set of clinical reports of children with bone loss, knee bone erosion, or joint pain after COVID, they call for monitoring bone age and growth velocity in children after infection - noting these complications may be underrecognized. @szupraha @ZdravkoOnline @adamvojtech86

  32. The study identified a dysregulated immune environment in Long COVID tissue in which the two branches of the immune system that normally work together – the innate and the adaptive immune systems – showed strikingly different responses.
    Innate immune cells showed continued inflammatory and viral-sensing activity, while the adaptive pathways needed to recognize & eliminate infected cells including antigen presentation, phagocytosis, and cytotoxic immune function were downregulated. The human gastrointestinal tract must allow contact with foreign materials (i.e., food) without triggering immune activation, and the SARS-CoV-2 virus may have immune evasion tactics to take advantage of this environment.
    The study’s tissue biopsy results are evidence that SARS-CoV-2 exploits, or is inadvertently protected by, the gastrointestinal tract’s uniquely tolerogenic immune environment that restrains adaptive immune activation (such as antibodies or T cells), while viral reservoirs continued driving innate immune activation (i.e., inflammation).
    “These findings suggest that Long COVID may involve a tissue immune environment that continues to sense persistent viral activity while failing to effectively clear virus-containing cells,” said Amy Proal, PhD, President and Research Director of PolyBio Research Foundation. 14/
    “The fact that these inflammation-driving reservoirs may be extremely low-abundance & unevenly distributed across tissues also underscores why persistent virus can be so difficult to detect.”
    Strikingly, abnormalities were far more apparent in tissue than blood.
    Researchers identified 240 significantly altered genes in colorectal tissue but only three in matched peripheral blood immune cells, suggesting that important Long COVID biology may be compartmentalized within tissues and poorly reflected in conventional blood measurements.
    The findings provide a rationale for treatment strategies that target both persistent viral reservoirs and the tissue immune dysfunction that may allow them to remain.

  33. Breaking: New Study Reveals Substantial Tissue-Level Immune Dysfunction Near Persistent Viral Reservoirs in Long COVID

    PolyBio-funded UCSF research finds persistent SARS-CoV-2 RNA alongside an altered gut immune response that may impair viral clearance
    Viral RNA was identified within the intestinal epithelium & also deeper tissue compartments. Because intestinal epithelial cells normally turn over within days, the presence of viral RNA in these cells months to years after initial infection is suggestive of ongoing viral activity or re-seeding of the tissue.
    Importantly, the researchers caution that the study likely underestimates the prevalence of SARS-CoV-2 persistence. Persistent viral RNA is present at extremely low abundance relative to human RNA, making it technically difficult to detect.
    Moreover, the endoscopic biopsy samples collected by the research team contain only a minute fraction of the ~25-30 foot (7-9 meter) long gastrointestinal tract.
    Viral reservoirs could therefore be focal, intermittent or concentrated in nearby or distant anatomical locations that were not sampled, meaning a negative colorectal biopsy does not rule out persistence elsewhere.
