This preliminary study suggests that circulating tumor DNA (ctDNA) may be a useful biomarker for predicting recurrence in patients surgically treated for early-stage esophagogastric #cancer. This study used the incremental cost-effectiveness ratio (ICER) to evaluate combinations of screening strategies for identifying Barrett's esophagus (BE) in the community, taking into account costs of sampling devices and prevalence of BE. Their main finding was that costs per BE case identified are minimized when using a weighted risk stratification model followed by less-invasive devices like the Cytosponge and then endoscopy.

This multinational study describes the development and validation of a diagnostic tool (EMERALD) -- a miRNA panel designed to differentiate persons with (or at risk of) #esophageal adenocarcinoma, from non-disease controls. The diagnostic potential of the panel was assessed using #machineLearning models. The findings suggest that it could serve as a #liquidBiopsy and facilitate screening in high-risk persons.

Due to its rarity there has been little research on how risk factors in the young might differ from older populations. Using data from the previously published BEST3 clinical trial this recent report estimated the potential for improving the detection of Barrett's esophagus (BE) and early esophageal adenocarcinoma (EAC) among individuals aged 50 and above with gastroesophageal reflux disease (GERD). Asbestos exposure, whether occupational or environmental, can cause several cancers, including mesothelioma and lung. A role in esophageal cancer has long been suspected. In this thorough meta-analysis, 48 cohort and case-control studies were analyzed, finding a clear association between occupational asbestos exposure and EC risk, particularly among asbestos product workers, asbestos applicators, and construction workers.

This study evaluates risk of death from causes other than esophageal adenocarcinoma (EAC) in patients undergoing endoscopic eradication therapy for esophageal dysplasia or early cancer. It finds that risk of death from unrelated causes is 40 times higher than that of EAC-related mortality, especially in younger patients. Post-EET follow-up protocols should consider this competing mortality risk in personalized management strategies.

Several studies suggest that bariatric surgery may lower risk of obesity-related cancers, but it's role in #prevention of #esophageal #cancer is less clear. A multinational Scandanavian study observed that gastric bypass surgery was associated with a decreased risk of esophageal #adenocarcinoma over time after surgery, compared to both the background population and non-operated individuals with morbid obesity.

This prospective study of a half-million persons in China observed a linear dose-response relationship between long-term exposure to fine particulate matter (PM2.5) and risk of esophageal cancer, using a satellite-based model to estimate exposure levels. If causal, PM2.5 may account for almost a quarter of cases in the area. These results highlight just one potential hazard from wildfires exacerbated by climate change.

This review gives insight into recent and developing approaches to the management of esophageal cancer, a malignancy that is especially difficult to treat. This study found lower risk of gastroesophageal cancer in persons who had bariatric surgery. Limitations include the lack of histological data, the young age of the participants, and the short follow-up duration. Cancers located in the lower third of the esophagus (where most adenocarcinomas arise) and the gastric cardia were somewhat more common in the surgical group, suggesting caution in the interpretation of risks for those cancers.

Radio frequency ablation (RFA) has become a standard of care for persons with advanced dysplasia in Barrett's, but little is known regarding the occurrence and predictors of post-procedure symptoms. This study found that RFA treatment caused considerable post-procedural symptoms of significant duration in the vast majority of patients, with almost all patients experiencing pain and dysphagia after the procedure.
