  1. Healthcare workers of various types are in short supply in Canada. It’s a key reason why we have backlogs in care. Lots of jobs. The province of BC on the west coast has made it much easier for healthcare workers from the US to move there. Other provinces are working on shortening the process too.
Heard about this source helping Americans learn more about moving to Vancouver Island. There was a great show on CBC Radio’s programme White Coat, Black Art hosted by Dr. Brian Goldman on this topic, interviewing the Maffins who created helpful info and sources.
There is also a group on Discord with discussions and where questions can be answered, also started by the Maffins.

  5. "Idk, I think it's less about empathy or compassion, and more about making sure the (hopefully small) group of uncaring people who enter medicine or allied health professions are incentivised to act pro-socially.
Like, in your region, what studying conditions and working conditions are clinicians expected to tolerate? Do they receive a living wage throughout study and work? Is their livelihood for a single person or big enough they can build a household?

Do they feel comfortable being a whistleblower when something goes wrong? (Numbers-wise, mistakes will happen over time.)
How robust is the inspection or regulation? How flexible are the logistics of each sector?
How many crises at once can be weathered without change to patient care? How long will it take to build back up that 'extra' capacity / contingency margin?

Who (in what position, in which teams) is responsible (or in what order) for noticing angels of death? What about anyone with less final but still harmful motives?

How severe are the consequences for failure? How many people feel compelled to hide errors rather than remedy them? What options for remediation are accessible to all staff, including contractors?"

  7. The claim is circulating that antidepressants can cause overheating and dehydration. This is a genuine public health message, but it's not so simple: diuretics, beta blockers, ACE inhibitors, antipsychotics, and some antihistamines may also interact badly with hot weather.
    nytimes.com/2024/06/15/well/li (excerpt in a reply)

There's even doubt about antidepressants having this effect. A recent paper in eClinicalMedicine looked at all the studies they could find and concluded that there wasn't evidence of antidepressants causing overheating in hot weather - though some other medicines do.

"Current evidence supports strong anticholinergics, non-selective beta-blockers, adrenaline, and anti-Parkinson’s agents impairing thermoregulation during heat stress ... physicians should interpret with caution conventional public health messaging related to the thermoregulatory effects of some drugs (e.g., antidepressants) during hot weather."

  11. I've had a hormonal IUS before, not a copper IUD coil.
jfc insertion is painful (but apparently not for other people??!) but not having to deal with condoms or the pill or anything? for YEARS?!

And the success rate is the highest or right up there. I think the arm implant is 1 pregnancy in 2000 couples over a year or two...? Unless that's the hormonal IUS.

ALSO THERE ARE DIFFERENT DOSES AND SIZES. If you're nulliparous (have never given birth) like me, GET THE SMALLEST, holy shit.
Why the flying fuck they bunged the massive Mirena in there, I've no fucking clue. Again: some people feel "mild discomfort" and other people do just feel some mild discomfort but not pain!

But yeah, even with the insertion pain, I'm getting an IUS again soon. It's just that damn convenient and reliable, I cannot lie.

  14. What are the issues with fascial plane blocks?

    • Non-robust research
    • Prolific researchers have had multiple retractions
    • Likely publication bias
    • Outcome measures are difficult to define
    • Plethora of 'novel' blocks make interpreting results difficult
    • Blocks don't cover visceral pain

Today we're talking adductor canal blocks for knee arthroplasty.

A recent UK survey showed that:
    • 89% favoured levobupivacaine
    • > 60% of these preferred 2.5 mg/ml and a volume of 20 - 24ml

  29. @drsimonpm
#deprescribing - music to my ears. All patients or their caregivers should be required to sit with a #pharmacist and perform a CMR (comprehensive med review) . Currently in the US, Medicare puts the onus on the pharmacist to contact (cold call) the patient. This is not ideal for obvious reasons. Of course the patient could seek our help voluntarily, but most are unaware of the value of such interventions. #Pharmocogenomics #MedMastodon
