Today, I'm chewing on Kate Manne's theory of misogyny as a social force for policing the roles of "human giver" and "human being" in what it has to say about the shapes of transmisogyny. I'm not sure if others have developed it in this way, but here's the thoughts it spurred in me.
It's especially this summary of Manne's framework in Nagoski & Nagoski's "Burnout" that got me thinking:
"Kate Manne describes a system in which one class of people, the “human givers,” are expected to offer their time, attention, affection, and bodies willingly, placidly, to the other class of people, the “human beings.” The implication in these terms is that human beings have a moral obligation to be or express their humanity, while human givers have a moral obligation to give their humanity to the human beings." (Nagoski & Nagoski, p. 8)
Of course, cis women are expected to be and socialized into the role of "human giver" while cis men are expected to be and socialized into the role of "human being," and if a woman should choose to act as a human being, rather than solely a giver, she is then punished - this is how the system of misogyny is enforced.

CW: The NHS - institutional issues - touches all forms of medical bigotry and prejudice
@jaelisp We're AuDHD, so trying to reconcile opposites is our daily struggle 😅
At a high-level, we see and resolve the seeming contradiction broadly as follows:
Nationalised health services, free at the point of use and paid for by taxation (national insurance contributions) are a fundamentally good idea at their core and have the capacity to be amazing.
That said, malicious forces will do everything within their power to erode any national public service, especially healthcare, for their own gain.
These malicious forces include (but are not limited to):
- Private healthcare companies.
- Free market capitalism ideologues / extremists.
- US-style Christian extremists.
- Fascists, racists, and bigots in general.
- Corrupt politicians (i.e., seemingly most politicians, sadly).
- Successive governments trying to make the service ever worse over multiple decades to justify slow, creeping privatisation.
It is not just being eroded from the outside either, sadly. There are ideological forces at work within the medical institutions themselves
We're talking (in no order) pervasive, insidious, institutionalised:
- Transphobia.
- Transmisogyny and misogyny
- Ableism.
- Fatphobia and sizeism.
- Infantilisation of neurodivergent folks.
- Lack of accessibility, especially on contact options, appointments, and support needs.
- Stigmatisation of mental health struggles.
- Queerphobia.
- Homophobia
- Biphobia and bi-erasure.
- Racism and xenophobia.
etc.
Even the BMA reported on the systemic issues recently!
Some issues are not the result of a lack of funding or expertise, but a fundamental refusal from the very top to change the way the system works.
On gender-affirming care, for example, The Transgender Issue: An Argument For Justice by Shon Faye breaks down how gatekeeping, long-waits, and transphobia were baked into the system from its creation.
Since its inception, access to trans healthcare has similarly been an ideological battleground. For those who need them, medical transition and contraception or abortion are – or should be – about the bodily autonomy of the individual, their right to mental well-being and the freedom to carve out their own destiny in defiance of prevailing gender roles. (These roles, should we need reminding, frame women as vessels for reproduction and trans people as threats to the strict separation of male and female sex roles on which patriarchy depends.) Access to abortion and access to trans healthcare are often attacked in similar ways: principally by overstating the incidence and likelihood of regretting either process, and an intense, disproportionate focus in the media on the stories of individuals who do regret their personal choices, as a way to undermine the principle of choice generally. Only about 5 per cent of women experience any degree of regret over their abortion. Multiple studies show the regret rate for gender reassignment surgery is even lower: about 0–2 percent. Despite this, the fear of regret has become a powerful tool used to justify the delay or withholding of treatment.
and
... it wasn’t until the 1960s that transsexualism became a formally recognized diagnosis within the British medical establishment. ... Medical support for trans people, however, was still rare. Throughout the 1960s and 1970s, individual trans patients continued to use the ambiguous and contested link between physical intersex traits and the psychological experience of gender dysphoria to get certain doctors to treat them – though, even then, few doctors would. A 1966 study in the British Medical Journal found that only ‘9% of psychiatrists, 6% of GPs, and 3% of surgeons’ would agree to actively assist transsexual patients. All of which effectively meant that trans people’s lives and destinies were dependent on the whims of a very small number of British doctors.
Shon goes into detail about the awful Dr John Randell, who worked in the gender identity clinic at Charing Cross Hospital, including one part that tells you all you really need to know about him:
Even when Randell had assisted with transition, patients often found him brusque, even cruel. ‘It hasn’t made you a woman, you know – you’ll always be a man,’ he reportedly told one trans woman who thanked him after surgery.
She sums up his pivotal involvement as follows:
It’s worth pausing to consider that the most powerful pioneer of trans healthcare in twentieth-century Britain was a cisgender male psychiatrist who believed neither in the reality of trans people’s deeply held identities nor that gender norms were socially constructed ideals that could be relaxed, challenged or abolished. He believed trans people were delusional about the reality of their situation and that at the same time they also needed to be highly competent mimics of gender stereotypes. He did not believe that they should be allowed freedom over their interpretation or expression of gender.
In short, the NHS could be great, but the current systems are fundamental rotten at their core. In order to fix the NHS, we would need to break both it and the medical institutions down, rebuild them, and then spend decades building them back up whilst engraining a culture of empathy, caring, and non-discrimination into the training of all staff (medical and non-medical) across the board.

It must become a moral imperative for the #dolls to fight, from the bottom up, with commitment and #fury. Fighting means #organizing, studying, being humble, not repeating the same dynamics that #cissexism imposes, and above all remembering that, as dolls, as #TransWomen, we are part, whether we like it or not, of a lineage of dolls before us who fought fiercely to give us the two rights we have, and that the community we deserve will never fall from the sky by divine grace, but we must build it ourselves, with our nails and teeth, in order to have dignified material conditions. For us european dolls, the US may seem far away, but those of us with our eyes open know that it is just around the corner.
We need practices and methods that are adapted to our conditions and do not seek assimilation. We need to learn to name the faces of our oppression because it goes beyond simple #transphobia. What dolls suffer from is #transmisogyny and as such it must be defined, just as all other facets need their own name. We need our own commitment because there are no messiahs or prince charming ready to save us.

Turns out a pol. left feminist friend, which I valued for her very solidarity w/ marginalised ppl, does exclude transfem* people from women. To her, they remain being men for alleged double privileges in life. I calmly disagreed to her but couldn't get into a debate.
My very first personal *terf*y experience after coming out and I just wasn't up for it, I felt completely overwhelmed. Luckily I had queer peers and compassionate allies to get to grips w/ it.
She's aware I don't identify as a woman, I'm a transfem* presenting enby. There's some trans*ness inside of me which I feel open & curious about to explore. You might imagine how on a personal level, her insolidarity with transfem*s, her reasoning, both devastated me personally and generally.

In a story that's not really making many waves in the US media because transphobia, transmisogyny, and hating incarcerated people are all bipartisan establishment positions, the Trump administration is also transferring incarcerated trans women to men's prisons; despite the fact that it's both illegal, and multiple judges have issued restraining orders to prevent the regime from doing so:
https://www.theguardian.com/us-news/2025/mar/07/transgender-women-prison-trump
Trans women transferred to men’s prisons despite rulings against Trump’s order
"Transgender women incarcerated in the US prison system have been transferred to men’s facilities under Donald Trump’s executive order, despite multiple court rulings blocking the president’s policy, according to civil rights lawyers and accounts from behind bars.
Trump’s day-one “gender ideology” order, one of several sweeping attacks on trans rights, said the attorney general “shall ensure that males are not detained in women’s prisons or housed in women’s detention centers” and that no federal funds go to gender-affirming treatment or procedures for people in custody.
The executive order was quickly challenged in court. In three lawsuits filed on behalf of trans women housed in women’s prisons, federal judges have ruled that the US Bureau of Prisons (BOP) cannot withhold their medical treatment and was barred from moving them to men’s facilities. One judge said the plaintiffs had “straightforwardly demonstrated that irreparable harm will follow."
Lawyers fighting Trump’s directive say the court rulings prevented the transfers of 17 trans women who are plaintiffs in the cases, but others not included in the litigation are now facing placements in men’s facilities."
In case you're missing the shell game going on here, the Trump administration is pretending that only the 17 specific trans women mentioned in the cases to block this cruel fuckery are covered, while simply going ahead to target other trans women who are not specifically mentioned in the court orders. Not only is this an abnormal, malicious violation of the spirit of the judge's orders, but it's also literally straight up illegal:
"Prisons are required under the Prison Rape Elimination Act (Prea), a longstanding federal law, to screen incarcerated people for sexual assault risk and consider LGBTQ+ status when making housing decisions. Legal experts say Trump’s blanket policy of housing trans women in men’s facilities clearly violates Prea."
This is a hard story to read, because it demonstrates a pattern of pointless, wonton cruelty towards trans women by Trump's Bureau of Prisons; in addition to moving trans women to men only facilities that put them in very real danger, the BOP has also issued *orders* for guards to deadname prisoners and use incorrect pronouns, change their gender markers on official forms, deny requests for gender appropriate clothing, and to allow male guards to pat down trans women prisoners. Furthermore because this administration (and some would say, American society) hates trans women more than they like ice cream, nobody is making any effort here to prevent abuses that would seemingly go even beyond Trump's fascist executive order. There are already reports of male guards "groping" trans women prisoners, prison officials seizing the bras and underwear of trans women prisoners like they were contraband, threats to force detransition incarcerated trans women by denying their hormone therapy, and even one report of a woman who had gender-affirming surgeries before her incarceration, being moved to a men's prison. As one lawyer noted, this "is essentially sanctioning sexual assault in some instances."
Of course, given that trans people represent a microscopic minority inside the U.S. population, and 17 trans women are being protected by temporary restraining orders, only a small number of trans women are subject to what I'd describe as a form of psychological torture and the constant threat of violence; perhaps that's why the US political and media establishment in the "opposition" doesn't care, rather than their well established indifference towards trans lives, if not outright transphobia. I would however argue that the fact that the Trump administration is plowing ahead to target such a small number of trans women demonstrates that this isn't about safety or the rule of law; the cruelty is indeed the point, and a Trump regime that cares little about malnourished children, seniors living check to check on social security, or American workers, is clearly prepared to move heaven and earth to make existence so intolerable for trans women that they simply stop existing.

"Identify as" language has been a disaster for trans people.
And besides the obvious, it also made it way more difficult to argue in our favor and counteract transphobic talking points.
You say "actually, HRT does such and such", and all the validity scolds come out of the woodwork to say "BUT NOT EVERYONE TRANSITIONS!"

We should be mindful about how we form our opinions especially on what is fair/unfair - a good start by LISTENING FIRST to the people most affected. It's been happening more and more, trans people, especially trans women, have been used as political punching bags and when we repeat "it's just biology" and such callous remarks, reducing the complexity of human beings this way, we are causing harm.

https://bsky.app/profile/kpt-san.bsky.social/post/3ku7ssb4qtc2x
