I'll be introducing a sentient, severed head, wearing the following vestige, in the next Theros session.
I wonder what the characters will do, and if any one of them will want to claim the diadem for themselves.

Who or what is the Triton they find in the dead Hydra's stomach?

Theros D&D concept: I love the idea of there being a mythic island in the Siren Sea, that, every 100 years or so, calls out to the fearless and mighty of Theros, asking one question, "are you strong enough?".
It draws them to the island, where a mysterious entity holds a tournament of champions, gathering the valorous and ruthless alike, to fight for glory.
Who knows what else might await the winner.

The characters are currently sailing along the shores of the Siren Sea towards Deyda Harbor (DH).
Little do they know that the "contents" of the Pyxis of Pandemonium, that they've opened, have begun to mobilize and will soon wash over Theros.
In a show of force, and as a threat to the PCs, the demons attack the harbour town. How do the characters become aware of this?

What monster am I going with for our first IRL Theros D&D session?
1️⃣ A territorial Hydra (huge / gargantuan)
2️⃣ A vindictive Warlock, their spider (gargantuan), and their brood
3️⃣ A haughty demon, delivering a "warning" via their minions (large) -

Card Name: Daxos of Meletis
Set: Theros
Description: Daxos can't be blocked by creatures with power 3 or greater.
Whenever Daxos deals combat damage to a player, exile the top card of that player's library. You gain life equal to that card's mana value. Until end of turn, you may cast that card and you may spend mana as though it were mana of any color to cast that spell.

Theors D&D poll: The characters witness as a star from Nyx falls into the ocean. These "Drops of Nyx" are highly coveted, and said to hold mystical properties (in meta terms: they hold 7th–9th level spells).
As the characters dive underwater to retrieve the object, what do they run into?

Okay, so I homebrewed a corresponding creature for the session. I might have to do some more fine-tuning, but I'm pretty satisfied with the framework so far.

Theros D&D poll: After investigating the ruins of the Lost Kingdom of Demons, finding a mysterious NPC and unleashing an ancient evil, the characters are back at a busy fishing village.
What can they expect from their visit?
A: Shopping, socialising and relaxation
B: A portentous, arcane storm assails the town
C: The Setessan guard has lost a "cub", and seeks aid
D: "Arachne", a mysterious NPC ambushes the group for unknown reasons -

Had an idea for Pharika's (Goddess of Affliction) temple. Over the centuries, many people suffering from illness have congregated around the temple in the hope of being cured by its acolytes. From this has sprung a settlement, "Loimos, the city of plagues".
Due to the suffering in the city, and the collective manifestation properties of Nyx, creatures connected to sickness and plague are known to appear in the settlement.

Theros D&D campaign poll: As the village was being destroyed by a titanic creature, two parents, whose discovery of a terrible secret (that threatened the gods) had inadvertently caused the attack, sought to protect their baby, and shield her from the wrath of the gods.
Who did they bargain with?

In our Theros campaign (mythic Greek setting), the party accidentally got a Paladin of Erebos (the deity of death) killed. Even though they didn't trust the Paladin and assumed she might betray them at some point, she was actually on their side and looked out for them.
The paladin was strong-willed, direct (to the point of rudeness) and somewhat manipulative.
If they search her body, they find a Bag of Holding. What is in said bag of holding?

CW: D&D Theros Poll
A Naiad (water spirit) guards a small lake in a forested region where ancient beings waged war millennia ago, the remnants of their cultures and buildings hidden among the undergrowth. A land once scarred by conflict, now reclaimed by nature, and altered by lingering magic.
What is the Naiad protecting?
