The stated end goal: training an AI therapy companion chatbot slated for release later this year, with plans to pursue insurance reimbursement for the automated tool.This is the business model. Clients provide intimate disclosures; the platform converts those disclosures into proprietary training data. These days, more people are willing to seek therapy, but the system meant to help them is failing. That is the contradiction at the center of a new interview with Psychotherapy Action Network Chair and Co-Founder Linda Michaels, PsyD, MBA, on WDET 101.9 FM's The Metro. The conversation is urgent, specific, and worth your time.For the first time in decades, more Americans are choosing talk therapy over medication. Stigma is declining. People are raising their hands and asking for help. And yet, as Linda makes clear in this interview, the structural forces working against them have never been more powerful or more coordinated.
Insurance companies already underpay psychotherapists by 20 to 30% relative to medical and surgical clinicians. The administrative burdens compound the financial penalty: joining an insurance network as a therapist can take five to six months of applications and follow-up. The same process takes about two weeks for a primary care physician. These are not accidents. They are the architecture of a system designed to make therapy an unattractive option for the people who provide it, which makes it an inaccessible one for the people who need it.
And insurance companies are not acting alone. In the interview, Linda names the full coalition bearing down on the field: private equity, venture capital, big tech, Amazon, Google. The global market for mental health apps reached $7.5 billion in 2024. These companies are not entering the field to improve care. They are entering it to extract data and generate profit. Psychotherapy Action Network's own research found that 85% of therapists said they would not work with a practice management company if they knew it was owned or heavily invested in by insurance companies. Nearly all of them are.
What happens when a small nonprofit advocacy organization challenges a well-funded tech company? Psychotherapy Action Network found out firsthand. When the organization raised concerns about a texting-based therapy platform's lack of evidence and its handling of patient data, the company responded with a $40 million defamation lawsuit. Psychotherapy Action Network fought it, secured pro bono representation on First Amendment grounds, and won the case in court. The episode is a window into how these companies behave and how much they’re are willing to spend to silence scrutiny, and why that scrutiny is exactly what this field needs.

Psychotherapy works because of what happens inside the room. But it also works because of everything that holds the room together.The psychotherapy frame, the structure of time, space, privacy, and trust that surrounds the therapeutic relationship, is not a formality. It is the clinical infrastructure that allows patients to take emotional risks, build trust, and do the deep work that creates lasting change. When that structure is destabilized, the therapy itself changes. For many patients, it becomes far less effective.
That frame is now under serious and organized threat. And Psychotherapy Action Network Chair and Co-Founder Linda Michaels has named it with exactly the clarity this moment demands.
Her new article, "Holding the Frame in an Era of Corporate Care," published in TAP, The American Psychoanalyst, examines how Practice Management Companies (PMCs) such as Headway, Alma (recently purchased by Spring Health), Rula, and Grow Therapy are quietly reshaping the conditions of psychotherapy, often without clinicians or patients fully realizing it until the damage is done.
Drawing on PsiAN's survey of nearly 700 mental health professionals, the article documents what is actually happening on the ground. Eighty-one percent of clinicians using PMCs reported concern that these platforms do not adequately protect patient information, including the potential use of clinical material to train AI systems. Eighty-four percent did not know how their reimbursements were split with the platform. Session limits, capitated payment models, and symptom checklists sent directly to patients without clinician knowledge or consent are increasingly common. And half of therapists who joined PMCs expecting to earn more ended up earning the same or less.
These are not administrative inconveniences. They are frame violations with real clinical consequences: higher dropout rates, shorter courses of care, erosion of the therapeutic relationship, and, ultimately, the false impression that psychotherapy simply does not work, with either the therapist or the patient to blame.
For patients, especially those with trauma histories or complex presentations, this instability carries particular risk. The populations most in need of clinical depth and reliability are precisely those most harmed when that reliability is compromised by corporate interests.
Michaels does not stop at diagnosis. She calls on insurers to immediately extend to all therapists the credentialing and payment improvements PMCs have already demonstrated are possible. She calls on clinicians to reclaim their authority over the frame and to recognize that business decisions are clinical decisions. And she calls on all of us, professionals and the public alike, to demand that the conditions for transformative care are protected, not sold.

This year, we are proud to recognize two teams whose work powers our work: our Communications Team and our Policy Team. These are clinicians, advocates, writers, lawyers, and administrators who give their time not because they have to, but because they understand what is being lost when psychotherapy is underfunded, automated, or pushed aside in favor of faster, cheaper, profit-driven alternatives. They understand what is gained when someone has access to a real therapeutic relationship that fosters lasting change.
Our Communications Team keeps PsiAN's voice active and visible. On our social media platforms, they write, post, engage, and translate complex clinical and policy issues into content that reaches members, the public, and allies across platforms. Their work is the connective tissue between PsiAN's mission and the public.
Our Policy Team assesses and analyzes the mental health landscape so that PsiAN can act with precision. They monitor legislation, track regulatory developments, and advocate for the policies that protect access to depth-oriented psychotherapy. In a field where the rules change constantly and the stakes are high, their vigilance matters.
Together, these volunteers represent something PsiAN holds as a core value: that the fight for transformative mental healthcare requires more than good intentions. It requires people willing to do the sustained, often unglamorous work of keeping an organization moving and a mission alive.
