Read Linda Michaels' letter in the New York Times. In it, the Chair and Co-Founder of Psychotherapy Action Network names something too much of the mental health conversation avoids: what's being sold as therapy often isn't therapy.
Writing in response to Daniel Duane's August 9 guest essay, Michaels draws the line. Therapy has imitators: platforms that dispatch counselors like takeout, TikTok influencers handing out diagnoses, coaching packages and chatbots rebranded as care. They borrow the language of therapy and the promise of relief. They leave out what makes it work.
What Sets Psychotherapy Apart
Psychotherapy rests on a relationship between therapist and client, the safety to question your assumptions and face your blind spots, and the privacy to do that work unobserved. The research Duane surveys keeps returning to this core: outcomes depend less on the particular technique than on the quality of that relationship. An emerging neuroscience framework he describes, predictive processing, offers a promising account of why, tracing how the brain updates its model of the world when a person feels safe enough to do so. It is not settled science, but it points where careful clinicians have long pointed.
Psychoeducation, self-help resources, and mental health content can inform and comfort. That usefulness is real. It is also not psychotherapy, and it should not be sold as an equivalent substitute for it. A chatbot runs a pattern of predictions; it does not hold a relationship. An influencer speaks to an audience; they do not know your history or carry clinical responsibility for your care. The relationship, the confidentiality, and the clinical judgment that give psychotherapy its power do not travel with them.
Who Profits When Care Becomes a Product
Many of these substitutes are backed by investors with their own priorities. Michaels names them directly: private equity, venture capital, big technology firms including Amazon, and the insurance industry, answering to quarterly statements rather than to the people they claim to serve. Some of these companies, she notes, want users' data to sharpen targeted advertising. More broadly, business models built on data or on high-volume, low-cost delivery can create incentives that pull against privacy and confidentiality, the very conditions psychotherapy needs to work.
The costs reach clinicians too. As insurance reimbursement rewards the fastest and cheapest treatment models, and platforms undercut sustainable practice, skilled therapists are pushed out of the profession at the moment demand is highest. Every therapist forced out deepens the access-to-care crisis, and the people who lose most are the ones who most need help.
Where Psychotherapy Action Network Stands
We reject the premise that therapy is a product to be optimized, automated, and sold. Psychotherapy is a human relationship that creates lasting change, and it deserves protection from the platforms, financiers, and insurers working to hollow it out. The distinction between psychotherapy and its imitations is not academic. It decides who gets meaningful care, who can afford to provide it, and whether the next person looking for help finds a therapist or an algorithm.

When a health insurer decides, without ever opening the chart, that the care a clinician documented and delivered was actually worth less, and pays accordingly, that is not a billing adjustment, it is a clinical determination rendered by a company that never met the patient. A growing number of commercial payers are doing exactly this. The practice is called Automatic Downcoding, and it is the latest way the insurance industry is quietly reworking the payment landscape of mental health care, this time aimed squarely at the physicians and nurse practitioners who provide it.
What Is Automatic Downcoding?Effective July 1, 2026, Health Care Service Corporation (HCSC), the fifth largest health insurer in the country, operating Blue Cross and Blue Shield plans in Illinois, Montana, New Mexico, Oklahoma and Texas, began automatically lowering the Evaluation and Management (E/M) codes physicians submit, without first requesting or reviewing the clinical documentation that supports them. The American Psychiatric Association (APA) reports that other commercial payers are adopting similar practices.
Psychiatrists and other physician-psychotherapists bill an E/M code that reflects the complexity of a visit, frequently paired with a psychotherapy add-on code when therapy is provided in the same session. Automatic downcoding means the payer reduces that E/M level on its own initiative and pays for a lower level of care than was provided, leaving the clinician to spend time appealing the decision to be paid correctly. Because no denial is issued, the reduction is often silent. It surfaces only as a practice's collections begin drifting downward, quarter after quarter, with no visible cause.
How Automatic Downcoding Hurts Clinicians and PatientsInsurers pay for less care than was actually delivered. This is a rate cut disguised as a coding correction.
It shifts the burden onto clinicians. The only recourse is to appeal, one claim at a time, on the clinician's own unpaid time. For solo and small practices, this administrative load may be too much to bear.
It presumes overtreatment. Psychiatric complexity is treated as suspect rather than a clinically real experience. This is the opposite of parity, occurring at a moment when behavioral health clinicians are already reimbursed roughly a fifth less than their medical and surgical colleagues for comparable work.
It relies on automation over judgment. These are algorithmic decisions made at volume, with no individual reviewing the encounter.
It threatens access. The uncompensated hours of appeals become one more reason for psychiatrists and nurse practitioners to narrow their panels or leave insurance networks.
A Pattern of Insurance Practices Undermining Mental Health CareThis is not an isolated policy. It belongs to the same pattern Psychotherapy Action Network has been documenting for years, which includes reimbursement cuts, session limits imposed by practice management companies, ghost networks, clawbacks, and unexplained rate reductions.
While there are relatively few physician-psychotherapists in the mental health field, this is not only their concern. The playbook is unmistakable: unilateral, automated payment reductions imposed without clinician input. What is aimed at E/M codes today is the same logic already reshaping how every mental health clinician is paid.

Real depth therapists don't simply offer validation at every turn. Therapists like this might help you feel good in the moment, but it's a thin veneer that won't last. At some level, we eventually feel the inauthenticity of these types of "support" in relationships...

Our Policy Team is looking to understand therapists’ practice management needs and their experience with therapy practice support platforms such as Headway, Ama, Rula, and Grow.
These companies offer a range of services for therapists, including insurance credentialing, billing, marketing, client matching, telehealth video, and an electronic health record systems (EHR).
We have developed a short survey to learn more about your experiences with these platforms.
