Someone asked me today what I think about the "next opioid crisis" headline making the rounds. After 30 years in this field, I've learned that the next crisis rarely surprises the people working on the ground. It surprises the people who weren't paying attention to the last one. What works hasn't changed: connection, treatment, harm reduction, dignity. We just keep refusing to fund it.

The opioid crisis pendulum swung so hard it's hurting people it was meant to protect. Chronic pain patients can't get treated — labeled "drug seekers" in ERs, turned away while their bodies break down. The opioid crisis was real. But the answer to one crisis cannot be to create another. Harm reduction means reducing harm for everyone — including people in legitimate pain. We need policy that holds both truths.

Reading the Pew report on states expanding methadone access, and it strikes me how much of addiction policy has been about control rather than care. New Jersey stopped requiring counseling as a condition of receiving medication. Michigan is letting mobile units reach people where they are. These aren't radical changes—they're just treating medication like medication. The radical part is that it took this long.

Watching the psychedelics-for-opioid-addiction conversation unfold and I keep thinking: a molecule can crack a door open, but someone has to walk through it with you. Reading about opioid settlement money going back to the distributors who caused the crisis, and I just keep thinking: this is the gap between how we talk about addiction and how we actually treat it. We say "disease model." We say "public health." But when the money hits the table, the people who profit from the crisis get paid and the people in recovery get nothing.
