Some of the largest publicly traded health insurers - United Healthcare, Elevance, Centene -- raked in the largest investment gains in at least a decade last year, according to a Modern Healthcare analysis of financial filings. These windfalls helped boost finances for insurers contending with headwinds such as higher-than-expected Medicare Advantage spending.

More than 55% of rural US hospitals no longer offer maternity care. In 2010, 46.5 million Americans had no health insurance. Today that number is close to 26 million. 8 percent of the population is still uninsured and many of the people who have health insurance still find it difficult to cover their share of their medical bills.
