If you're ever looking for a GDPR-compliant form and survey tool I can not recommend Tally enough. It's built and managed by indie developer couple Marie & Filip in Belgium.
99% of features are available for free. But yes, I pay the very affordable price of €250 per year.
Remember though, GDPR compliance doesn't stop with the tool you choose, it also depends on what you ask for, how you ask and how you manage the data after collecting it.
