    TITLE: STAT+: In the battle of sepsis algorithms, performance alone doesn’t predict victory

    Five years ago, the bottom fell out of sepsis prediction software. Hundreds of hospitals had adopted an algorithm from electronic health record company Epic that promised to alert physicians to predicted cases of sepsis, a life-threatening reaction to infection that kills more than 350,000 people in the United States every year. 

    The AI was a technical flop. Despite its results on paper, the technology failed to perform in the real world, and sent so many alerts that doctors tuned them out or hospitals turned them off. 

    Half a decade on, new sepsis models are hitting the scene. Epic released a retooled version of its own algorithm. Startups are testing their models in health systems. A team uses large language models to mine clinical notes for signs of sepsis. And on Tuesday, a sepsis flagging device from Bayesian Health, with origins at Johns Hopkins, announced it has received clearance from the Food and Drug Administration.

    TITLE: Opinion: AI doctors should be licensed. Here’s a framework to do that

    Late last month, Utah’s Medical Licensing Board called for the immediate suspension of the state’s pilot program with the AI company Doctronic. The program lets a chatbot evaluate patients and recommend prescription renewals for nearly 200 chronic condition drugs, with the state planning to phase out physician review of each case.

    The board said that it only learned about the pilot after it had launched. Its warning was blunt: proceeding without proper clinical oversight “potentially places Utah citizens at risk.”

    Roche has signed a deal to pay $750 million upfront for Boston-based PathAI, an acquisition by the Swiss pharmaceutical giant to speed up its use of artificial intelligence to help pathologists diagnose disease.

    The agreement, which is expected to close in the second half of the year, could generate an additional $300 million for PathAI if it leads to the achievement of certain milestones.

    “Joining forces with Roche marks a new era for PathAI, enabling us to realize our mission of improving patient outcomes through AI-powered pathology at unprecedented scale and speed,” said Andy Beck, chief executive and cofounder of PathAI, in a statement. “Roche’s global infrastructure and expertise will bring our digital diagnostics technology to patients worldwide.”

    In late April, the Centers for Medicare and Medicaid Services and the Food and Drug Administration announced the Regulatory Alignment for Predictable and Immediate Device, or RAPID, coverage pathway. On paper, it is exactly what the medical device community has been asking for: a synchronized process that could deliver Medicare national coverage as soon as two months after FDA market authorization, rather than the year or more families and manufacturers currently endure.

    I want this pathway to succeed. I have spent more than a decade helping small companies bring novel devices through FDA review, including several that earned breakthrough device designation. I have watched reimbursement delay strangle technologies that children desperately need. Faster, more predictable coverage is a real problem, and RAPID is a real step.

    But this announcement does not fix the way pediatric and orphan devices chronically lag behind their adult counterparts. In some ways, in fact, it deepens that gap.

    TITLE: Fraudulent citations, blamed on AI hallucinations, are becoming more common in research papers

    Citations in academic papers are intended to ground research in the work that preceded it, over time creating something of a family tree explaining the roots of ideas, protocols, and studies. 

    But a growing number of these citations lead to dead ends. “Fabricated” citations that do not reference real papers are spreading in the literature, polluting the public record of science, a new study published Thursday in the Lancet shows. Tools using generative AI are likely to blame, say the Columbia University researchers who authored the paper.

    TITLE: STAT+: Color Health moving deeper into cancer services, complete with virtual ‘tumor boards’

    The way Color Health’s CEO Othman Laraki sees it, cancer has a scaling problem. New science regularly sets new standards of care, increasing the intricacy of managing an already complex illness. Cancer patients are multiplying faster than oncologists, Laraki said, and costs, too, are exploding. All this makes it difficult for everyone to receive the best possible therapy. The solution that the Silicon Valley executive sees is inevitable.

    “In our mind, the only way this is going to be addressed and solved is in a virtual first, AI-driven manner,” Laraki said. “In the coming years, the biggest cancer centers will be virtual first.”

    Virtual care for cancer may sound like an oxymoron. After all, the pillars of cancer treatment are almost all hands-on: surgery, radiation, infusions, and the like. But Color Health has been building out a virtual cancer clinic — including a virtual “tumor board” of multidisciplinary experts —  that the company says can deliver and manage care at a high quality. The company just received a certification from the American Society of Clinical Oncology to back it up.

    TITLE: STAT+: OpenAI wants to ‘have their cake and eat it too’ with health AI policy recommendations

    So far, artificial intelligence company OpenAI’s work in health care has been largely limited to nonregulated areas. In January, the company released ChatGPT Health for consumers. Then, it launched ChatGPT for Healthcare for hospitals, followed by ChatGPT for Clinicians. 

    Along with the launch of its chatbot for clinicians last month, OpenAI also published a wish list that the company described as a blueprint for unlocking AI’s potential to change the broader health care system. The policy proposals outlined in the document, experts told STAT, are somewhat reasonable, but they also disproportionately benefit the company. 

    “They’re trying to have their cake and eat it too,” said David Blumenthal, a former national coordinator for health IT and a health policy professor at Harvard University. “They’re trying to potentially sound like responsible parties in the current conversation while at the same time wanting the markets to stay open for their products.”

    TITLE: STAT+: As artificial intelligence show off diagnostic chops, scientists reckon with the way forward

    Getting a paper published in Science is a highlight of many researchers’ careers. But for internist and clinical AI researcher Adam Rodman, it’s also been a source of some agita. 

    On Thursday, Rodman and his colleagues published a compilation of experiments, including one using real-world data from a Boston emergency department, that show a large language model from OpenAI can outperform physicians in case-based diagnostic and clinical reasoning evaluations. To Rodman, the paper’s co-senior author, it’s a response to a gauntlet thrown down in Science in 1959. That paper “described how you would know that a clinical decision support system was capable of doing diagnosis better than humans,” he said. “And they can do it.”

    But as generative AI tools like chatbots are heavily marketed — both to patients and clinicians — it makes him worried that the science experiments, all based on simulated and historical cases, will be misconstrued as proof of AI’s safety and efficacy when used to treat real patients.

    The U.S. Food and Drug Administration has proposed excluding the active ingredients in wildly popular obesity and diabetes drugs from a list of substances that can be used for making certain compounded versions.

    The agency determined there is no “clinical need” to allow large compounding facilities to make large quantities of medicines from semaglutide, the ingredient in Novo Nordisk’s Wegovy weight-loss and Ozempic diabetes treatments, as well as tirzepatide, the ingredient in Eli Lilly’s Moujaro diabetes drug and Zepbound obesity medicine.

    The decision singled out these compounders, which are known as 503B facilities in regulatory parlance, after growing controversy over their role in making weight-loss treatments available over the past few years. In explaining its decision — which is a victory for Novo and Lilly and rework choices for consumers — the FDA explained these compounders no longer meet the legal requirements to market their products.

    In his recent video “The People Do Not Yearn for Automation,” The Verge editor-in-chief Nilay Patel explains “software brain” — thinking about the world as a series of databases that are easily manipulated to solve problems — and why that is creating a disconnect between the AI world and everyone else.

    The AI world thinks that AI really can solve the world’s ills and thus anti-AI sentiment is just a marketing problem. But people who are trying to slow AI adoption have legitimate concerns about the tradeoffs and performance of the technology that software-brained people are dismissing, he says.

    As Google faces pressure to take greater accountability for the mental health impacts of its artificial intelligence products, the company’s clinical director Megan Jones Bell welcomed the challenge of making artificial intelligence helpful to people who come to its Gemini chatbot with a mental health crisis.

    “It can seem sometimes like shutting something down is a way of preventing harm,” Jones Bell told STAT. “We believe that making our product experience safer and more helpful and strengthening that bridge to support it is the more effective path to support mental health for the most people.”

    Google recently made updates to its Gemini app so that it more prominently features connections to crisis hotlines when it detects a person may be at risk of self harm. In conversations about mental health, the AI will frequently point people to outside resources — but the bot doesn’t disengage, reminding a user, for example, that “I’m here to listen.”

    This is Jacob Robinson’s vision — and it got closer to reality on Friday after the Food and Drug Administration approved a request from Robinson’s startup, Motif Neurotech, to start an initial feasibility trial to test the efficacy of their device in treating depression that hasn’t responded to other treatments. Scientists have been zapping brains to alleviate depression for decades through a method called transcranial magnetic stimulation, or TMS. Motif wants to do the same thing, but with a twist.

    Utah’s high-profile experiment with using an artificial intelligence system to renew prescriptions without physician oversight is facing its first major challenge as doctors in the state push back.

    In a letter published Friday, Utah Medical Licensing Board said it only learned about the pilot program with AI doctor startup Doctronic after it had been launched, and asked the state to halt the program.

    “Proceeding with this agreement without consulting the Medical Board potentially places Utah citizens at risk and remains a major concern of the board,” they wrote. “It is the strong recommendation of the Utah Medical Licensing Board that this program be immediately suspended pending further discussion.”

    Federal regulators on Thursday announced a new Medicare coverage pathway for devices deemed breakthroughs by the Food and Drug Administration. The program aims to address longstanding industry complaints that it takes too long for Medicare to pay for innovative technology.

    The new Regulatory Alignment for Predictable and Immediate Device coverage pathway, or RAPID, will try to synchronize the FDA’s premarket review process, which determines whether products are safe and effective, and Medicare’s independent process which determines whether paying for devices is reasonable and necessary. The goal is to collect and review enough data before authorization that Medicare can cover the products soon after a product hits the market. It’s the latest effort to speed up patient access to breakthrough devices.

    “We’re going to demystify the process by providing a clear signal to device manufacturers of exactly what targets they need to hit to get Medicare coverage,” said Medicare Deputy Administrator John Brooks. “And that’s enormously valuable, if you think about the investment timelines for device manufacturers who are trying to bring life-saving treatments to individuals.”

    The little pink pill recently got a retail boost. Addyi, a drug that treats low libido in premenopausal women, has been on the market for more than a decade after a controversial approval. But in December the Food and Drug Administration expanded its use, approving the daily drug for all women under 65. 

    More women than ever now could qualify for the pill — and its maker, Sprout Pharmaceuticals, made it easy for them to find out. On its Instagram account, in Facebook ads, and on its website, Sprout shared how women could get a quick consult for an Addyi prescription: With a special code, PINKPILL, they could get a telehealth visit for just $10. 

    Drugmakers have long used discount coupons to encourage patients to use their high-cost medications. Now, coupons can influence not just a drug’s out-of-pocket price, but the cost of consulting with a clinician who can prescribe it. 

    For Addyi, those consultations and discounts are managed by a telehealth company called Prescribery — one of several that work with drugmakers to help patients “talk to a doctor now” about their products. “We give them the coupon codes that they can use, and they get to market it to drive additional business,” said Prescribery’s CEO and CFO Ross Pope. “That’s sort of our arrangement, where they’re driving more business, both for them and for us.”

    As links between pharmaceutical companies and telehealth providers grow, health policy experts and legislators have sounded alarms. Telehealth companies can receive hundreds of thousands of dollars in pharma fees a year. Critics have questioned whether those partnerships break federal laws prohibiting financial kickbacks to induce prescribing, highlighting their potential to promote uncoordinated care and overprescription of unnecessary, and often expensive, branded medications. 

    The same questions, they say, apply to coupons extended for a drug-specific telehealth visit. “These discount structures are one more piece of the same puzzle,” said Daniel Eisenkraft Klein, a postdoctoral fellow at the Program on Regulation, Therapeutics, and Law at Brigham and Women’s Hospital and Harvard Medical School. “It’s this big financial architecture that’s every step of the way designed to move patients toward the specific drug cheaply and quickly.”

    To hear health systems tell the story, artificial intelligence tools like ambient scribes are helping not only reduce doctor burnout, but also increasing payments from insurers that haven’t been compensating them properly. But on insurer earnings calls, the payers position themselves as white knights sounding the alarm on providers using AI to raise health care costs to an unsustainable level.

    However, at least behind closed doors, both sides appear to agree that AI scribes are driving up health care costs. 

    “The investors, the health plans, and the providers, in private, were like, ‘OK, well, it’s quite clear scribes are increasing coding intensity. One hundred percent,’” said Caroline Pearson, executive director at the Peterson Health Technology Institute, describing a roundtable PHTI held earlier this year. The nonprofit institute, founded in 2023, evaluates the impact of new technologies on health care costs and quality.

    They remind you to not talk during the movie, but when I saw “Project Hail Mary” last week, I was so alarmed that Ryan Gosling placed two Eppendorf tubes next to each other that I said, aghast, “He didn’t balance the centrifuge — that would have wrecked it…” while my non-scientist friends cracked up.

    That one scientific flub aside, it’s still one of the best book-to-film adaptations I’ve seen, maybe ever.

    TITLE: STAT+: AI drug developer Insilico Medicine and Lilly ink commercialization deal worth up to $2.75 billion

    Alex Zhavoronkov, CEO of Insilico Medicine, can’t stop complimenting Eli Lilly. “Lilly is better in AI than Insilico, and no other company is better in AI than us … except for these guys,” he said. 

    He insisted he wasn’t saying nice things about Lilly just because the pharma giant has signed a new deal with Insilico that’s worth $115 million up front and approximately $2.75 billion in biobucks, which are contingent on achieving regulatory and commercial milestones. After calling Lilly’s tirzepatide, which he is on, “the best drug ever invented by humans,” he said he’s been consistently singing Lilly’s praises for a year. “Mounjaro makes me so happy every day. I want to develop the next one.”

    It looks like Zhavoronkov might have the opportunity to do just that — his AI drug development company’s new deal with Lilly, announced on Sunday, includes rights for the Mounjaro and Zepbound manufacturer to develop, manufacture, and commercialize some of Insilico’s preclinical AI-discovered candidates for oral therapeutics. Though he declined to say which assets Lilly licensed, he said that the company is the “absolutely best partner” for the candidates and that “nobody is better than them” in these disease areas. Insilico’s pipeline webpage recently was updated to note that a candidate targeting GLP-1 has been out-licensed to an undisclosed partner.

    ‘World model’ developer tied to Nabla gets $1 billion

    Advanced Machine Intelligence (AMI), the new company from former Meta chief AI scientist Yann LeCun, announced it had raised $1 billion for its quest to develop “world models.” This is a health tech story because AI documentation company Nabla will have early access to this new technology as it aims for an edge in a crowded field of companies trying to automate complex and laborious health care work. AMI’s new CEO Alex LeBrun is the co-founder and CEO of Nabla, and LeCun is one of Nabla’s investors. There’s no formal equity or licensing agreement relationship yet between AMI and Nabla, but the companies are already working closely together, Nabla COO Delphine Groll told me.

    TITLE: STAT+: Patient health data as a public utility: A former ARPA-H data chief explains

    Last year, the Department of Health and Human Services published a sweeping document that described the agency’s approach to real-world data. Historically, health and biomedical data has been intentionally manufactured, the output of carefully designed clinical trials. But in a digitized world, it can instead be mined — and patients’ interactions with the health care system are the natural resource.

    The Living HHS Open Data Plan, published in July, proposed treating data more like we do other natural resources. “At the core” of the plan, it reads, “lies the concept that data is a ‘public utility’ for good that powers scientific advancement, innovation, and progress.” Patients should have access to that utility, HHS argued, but it should also be easier to leverage for research, safety monitoring, and other uses in the public interest. 

    On Thursday, a group of researchers, former agency officials, and health data companies continued that call in a policy forum published in Science. If health data is to be treated like a public utility, they write, it should be similarly governed. Like electricity, the system would have to involve customers, local distribution companies, transmission companies, generators, and the government.

    TITLE: STAT+: Epic launches AI Charting, potentially scrambling the ambient scribe market

    The nation’s largest electronic health record company, Epic Systems, today announced more details around its long-anticipated AI Charting feature, which could disrupt the market for ambient scribes. Several health systems are piloting the tools now, the company said.

    “I almost bristle a little when people are like, ‘Oh, it’s an AI scribe,’ and I’m like, ‘No, no it’s a scribe, but that’s passive. We really want this to be active,’” said Jackie Gerhart, chief medical officer at Epic, in a call with STAT ahead of the announcement. “We’re just going to keep adding more besides just notes. So it’s going to be orders, it’s going to be diagnoses.” 

    Epic is hardly the first mover in the ambient scribe space, but given the company’s dominance in the EHR space it can  leverage its huge platform and armory of patient records as the reason its product is superior to others. Epic holds 42% of the acute hospital EHR market and 55% of beds, with the next-largest company, Oracle Health trailing behind at 23% of the market and 22% of beds.
