Haemophilus influenzae invasivo en España: el aumento de 2024 no se concentra en la infancia

El Boletín Epidemiológico Semanal acaba de publicar los datos de enfermedad invasiva por Haemophilus influenzae(EIH) correspondientes a 2024. Se notificaron 769 casos, con una incidencia de 1,58 por 100.000 habitantes, un 12,3% más que en 2023 y la cifra más elevada del periodo estudiado.

La lectura pediátrica, sin embargo, es distinta de la cifra global. Los menores de un año continúan siendo uno de los grupos con mayor incidencia —8,68 por 100.000—, pero esta disminuyó un 7,9% respecto a 2023. También descendió claramente entre 1-4 años (-58,7%) y 5-9 años (-70,2%). El incremento global se concentra fundamentalmente en edades adultas y avanzadas.

Entre los casos con información clínica, la neumonía bacteriémica fue la presentación más frecuente (49%), seguida de meningitis (15,9%) y sepsis (14,2%). La meningitis representó el 26,3% de las presentaciones conocidas en menores de un año.

Un dato particularmente relevante es que solo se obtuvo información sobre el serotipo en el 36,7% de los casos. Entre los tipados, el 76% correspondió a cepas no capsuladas. Esta baja exhaustividad impide sacar conclusiones firmes sobre la distribución real de cepas y obliga a interpretar con prudencia los casos de enfermedad por Hib observados en niños vacunados.

Los datos europeos de 2024, publicados recientemente por el ECDC, muestran un patrón concordante: aumento de la EIH, máximas tasas en menores de un año y personas mayores y predominio de cepas no capsuladas entre los aislamientos caracterizados. ECDC

La vacuna frente a Hib sigue incluida en España a los 2, 4 y 11 meses. El mensaje que deja este estudio no es cuestionar la vacunación, sino reforzar la vigilancia y, especialmente, mejorar la caracterización microbiológica de los casos invasivos. Ministerio de Sanidad

Para llevar a la consulta: la EIH está aumentando en España, pero los datos de 2024 no muestran un aumento paralelo en la población infantil. Los lactantes continúan siendo un grupo especialmente vulnerable y la información disponible no permite afirmar que exista un rebrote pediátrico por Hib.
