My hubris has cost me.
After my successful siege defense, I sent my veteran squads to retaliate and raze one of the smaller goblin pits.
Unfortunately while they were away a fierce dragon got in before I could close the gate to the caverns.
I lost over 70 dwarves before it was finally killed. The brave dwarf who killed it died of his wounds afterwards.
