The Doctor correcting the way The Clown holds a scalpel reminds me of Robert Knepper's character, Dr. Peter Kelmer, would talk about the right way to hold a scalpel in the Law & Order: Criminal Intent episode "The Good Doctor", even correcting Det. Robert Goren.
Robert Knepper appeared as Wyatt Miller in the TNG episode "Haven" and as Gaul in the VOY episode "Dragon's Teeth"; both, before the aforementioned appearance on LO:CI and well before portraying Samuel Sullivan in the fourth and final season of Heroes.
