I'm reminded of the first HeLa failure-of-science scandal - contamination judgement failures that have been pretty much completely obscured by the later ethics issues.

The first had multiple laboratory heads all acting on the basis that contamination of the tissue cultures they were using in their cancer research (lung cells, liver cells etc) by the highly proliferative, very robust HeLa cervix cells they were using as well, "could not happen in my lab"
