What. A. Week!!! It started all "as usual", until that one call, Monday morning around 10:45. It set off more calls... And then stress and anxiety. And then fear and doubts... On Tuesday, I had my Trapeziectomy surgery. All went well, or so they said. Wednesday, I felt so slow and sore... I tried to make things work out, and care as best as I could for Koa. I got some very sweet help from the neighbor, who took Koa and myself for a nice, and well needed walk. On Thursday, a sweet friend came over to help me with some garden work. It had been planned before we knew about the OP, so we kept the appointment, and we'd see how far the both of us could get. The rest of the week was challenging. I struggled with pain, nausea, being so exhausted that I couldn't even fall asleep properly. I tried to keep going. I tried my best... Cracks are where light gets in
I’ve been seeing the quote: “When you are healthy, you have a thousand problems; when you become sick, health becomes your only problem.” It’s true, but it’s not a reality any of us wants to live through.
2026 has been an eventful whirl, defined by health issues. My youngest, Emilia, already underwent three hospitalizations this first half of the year alone.
April: Diagnosis and Tests
It started with severe abdominal pain, which she rated a 10/10. She couldn’t sit up from the pain, and it turned out she was suffering from pancreatitis that got her confined for a week. The root cause was choledochal cyst, which is a congenital condition that hadn’t shown up on her full ab ultrasound in 2025 and past tests since she was younger. She had basically been misdiagnosed with gastritis and a sensitive stomach her whole life. But this 2026, her new gastropedia spotted it on an abdominal X-ray, and a follow-up CT scan revealed it was already the size of a chicken egg, measuring 6cm.
The only definitive fix was major surgery so it wouldn’t recur. Pre-surgery clearance became our next hurdle.
We ran a series of pre-op tests: blood work, X-rays, and an MRCP with IV contrast: a specialized MRI using dye to map her bile ducts. The challenge was finding a hospital with both the dye and a pediatric body coil. Thankfully, my cousin who used to work at The Medical City helped us secure a slot after a one-week queue, which was practically a fast pass.
When I asked her doctor for a ballpark cost since HMOs don’t cover congenital conditions (which is different from pre-existing), she gave it to us straight: at least ₱250,000. (And we’ll discover later on that it meant professional fees pa lang pala.)
May: Research and Selection
When it came time to pick a hospital, her doctor strongly endorsed Philippine Children’s Medical Center (PCMC). Their entire team is used to operating on tiny, delicate bodies, which makes all the difference.
We were no strangers to PCMC, but this time I was after a specific pediatric surgeon. His name was the constant recommendation from pay-side secretaries and clinic security guards. I looked him up, read his published international medical journal papers, and saw his case studies on choledochal cysts. So I decided he’s the only choice.
June: Surgery
Our June 7 admission slipped to June 12. We were admitted late in the evening, with surgery scheduled for 11:30 AM the next day. Painkillers were gradually administered through her IV to prepare her.
At 11:20 AM, we were wheeled to the Operating Room. They asked me to put on a hospital gown to accompany her inside. I like that protocol. I think it reassures kids that their loved one is with them, at may naghihintay at babalikan sila. I squeezed her, told her that I love her, and that it would all be over when she wakes up. They placed the anesthetic mask over her face. She passed out instantly, and the last thing she saw was my face, probably speaking gibberish, remembering my own past surgery where words turned incomprehensible before everything went dark.
I cried out of sadness, pity, worry, yet also relief that she wouldn’t see me. I wanted to stay even if I knew I would be a liability and distraction. I knew that, but it was my child lying on the operating bed. A staff member gently guided me out.
OR security told me to stand by for 3 to 4 hours. Time stretched endlessly. I spent hours in tears that won’t seem to stop. Jap stayed steady, probably because I was a wreck that he had to piece himself together for both of us. Friends, family, and colleagues messaged for updates, but I had nothing to offer except my panic.
By 4:00 PM, the fifth hour, a new guard took over the shift, reminding us just how long it had been. My in-laws were there. My thoughts were spiraling, so I approached another parent to talk, just to pull my mind off Lia for a bit.
“Zafra,“ the guard finally called out at 7:30 PM.
She said our daughter would wake up soon and needed one of us. I threw on the gown and went to the recovery room. As the anesthesia wore off, I watched Lia open her eyes wide. She was looking at me, but also beyond me. She said later on that she couldn’t make out my face then, but she knew. She looked so small, thin, and battered.
Through it, I was also in awe of anesthesiologists because imagine that precision of calculating age, weight, and dosage down to the exact detail to put a body down safely and ensure they wake up at the crucial point post-op. It must be satisfying and fun to pull off.
Three tubes were attached to Lia: an NGT through her nose to drain bile, a Jackson-Pratt drain for post-op fluid, and a urinary catheter. She had undergone a massive procedure:
- Removal of choledochal cyst, gallbladder, and stones
- An open Roux-en-Y hepaticojejunostomy (reconstructing her digestive tract by connecting her intestine directly to her liver’s bile duct in a “Y” shape)
- Biopsy (long story short: benign!)
We stayed in the hospital for 11 days. The final bill was the costliest paper I have ever held in my hand. Of course, I cried out of helplessness. (Like, may magpuputa na ba sa’min? And si Jap ‘yun, if it comes to that because as a mother, I should be a role model to my daughters. ~I said what I said, but I’m kidding.)
What really spiked the bill were the professional fees. Early on, the surgeon said he’d be fine accepting Guarantee Letters, which gave us a bit of breathing room. But things happened. He later explained that PCMC management had tightened their rules and stopped allowing GLs for pay-side patients.
My insurance policy, paired with what little savings we had and help from our circle, was the only reason we didn’t have to swipe the entire bill on a credit card. Still, it’s a policy loan, and we’re sitting on a debt while balancing monthly amortizations for the car and the lot. I’m a sandwich provider, supporting my aging parents alongside our own family. My sister and I split our parents’ monthly maintenance meds, while Jap and I handle all household bills and groceries. Plus, there’s the quarterly premium for our life insurance na hindi mura ha.
God, with audacity: “If it’s Your will, it’s Your bill.“
July: Stomach Flu and Colign’s Birthday
On July 25, a basic stomach flu hit Lia. But at one month post-op, it was alarming. By 4:30 AM, she was vomiting clear fluid and food particles alongside diarrhea. By 7:30 AM at PCMC triage, she had already vomited seven times and pooped six.
They ordered an abdominal X-ray, blood tests, and another NGT insertion to drain her gastric juices and rule out a post-op infection. To be cleared, she had to graduate through a strict regimen: Nothing by Mouth (NPO) > to clear liquid > then soft food > and finally solid food—all without pain, vomiting, abnormal NGT drainage, fever.
I wasn’t as scared this time because we had already been here. I didn’t mind the waiting and taking turns either sleeping on a plastic monobloc chair to watch over Lia or sleeping outside in the car (that was the easier choice because you could recline).
We were there for three days. We watched doctors and nurses pull brutal 24-hour shifts back-to-back, all while remaining sharp. And out of pure chance, one of our attending ER doctors was even my journalism batchmate.
We’ve seen and slept beside critically ill kids in the ER before, but it was our first time hearing a child’s time of death being announced behind the next curtain. I told Lia what happened, and she cried, and I instantly regretted telling her. They called for the mom, who entered the room wailing. It was a depth of despair you could witness but could not fully describe. Which is why you don’t ask: “Why us?” because that implies it should happen to someone else instead. You just pray to be equipped. To heal. To be stronger. To push through.
I’m pushing 40. I had reached a point where life had finally stopped feeling like a race, where I thought providing a comfortable life and staying calm was enough. But I’m realizing that’s just complacency given our status because it’s pitiable how this wiped out our savings. And this is just with my daughter. What if it happens to my parents, who have no HMO, insurance, or savings? What about Jap’s parents? What about us? We have to upgrade our capacity because at the end of the day, we are the safety net.
Still, I’m optimistic. I’d say toxically optimistic, even. Because it’s not hard to believe things will get better when your baseline started so low.
What I’ve been feeling lately is a complex mix. I’m discontent, but I’m grateful. I tend to rationalize, but I’m also actually pronoiac. I know hard work and grit matter, but I’m also fully aware of the luck, generosity, and a bit of privilege that carried us.
Good things still happen. In fact, we got approved for zero balance billing even if we previously came from pay side (which is a crossover they explicitly said isn’t allowed). Yet we got approved. Maybe because the hospital noted her recent surgery was just in June, which would be indicative of out-of-pocket costs we shelled out recently. Or else, baka matuluyan na talaga tayong maging parte ng 4Ps. There was also free food and little take-home gifts for the kids on our last day.
Actually, July was also Colign’s birth month. When I asked her how she wanted to celebrate, all she asked for was a good space to practice skating. So we spent the day at La Mesa Ecopark, marking our first outdoor family bonding since Emilia’s surgery. Colign is currently hooked on the Persona game series, so we gifted her a Shujin Academy school bag from Persona 5. And since she loves Chinese food, we ate at Shi Lin afterwards, where my sister and Gian joined us.
Good things continue to happen.
I know some form of generational trauma has been passed on when my daughter’s go-to is picking the cheapest option or turning down gifts altogether. Colign is the child who lived through those lean years with us. Lia is entirely different though. If I say I’m not buying, she’ll retort with “pero may pang Shopee sya.” 😂 😭 -
