  1. What has stopped Democratic states and localities from providing public health insurance to their workers and their families instead of dealing with private health insurance companies?

(This question is not asked with snark.)

It seems like people being able to treat themselves and their families early on in a health problem would save a ton on healthcare costs in the longterm, which actually ought to matter to public entities.

A city/County like Los Angeles is bigger than most US states, and a state like Washington could make up for some of that by combining state employees with public employees of willing cities and counties.

This is not a "Are they stupid??" question. It's a "what issues (other than lack of political will by Democrats) make this more difficult than it sounds?"
