Had missed this till now. What a tragedy.
At first glance has some uncanny similarities to the death of Khyra Ishaq in 2008, which I did a long & detailed analysis of in 2010. In Khyra's case too, numerous adults were worried for a long time beforehand, and social services didn't use what powers they had.
