We reach Rabaul after 4 days of navigation over the Bismarck Sea. I am now docked at the Kute shipping quay for how many days I don't know yet. The good thing is that there's a new company covering the last stretch of my journey, from Rabaul to Buka, Bougainville. I'll just have to wait for the time the ship is ready to depart. In the meantime, I watch at slave-like work of a group of men downloading copra from a cargo ship and uploading it on trucks delivering it on local markets. They tell me they get the equivalent of €2 for every ton of copra downloaded, to be divided among a team of 6. Slavery I said.

However, people leave me some space to roll out my 2-inch mattress on the floor (I'm the only one onboard except for crew members enjoying this luxury), so I get some rest. All other passengers sleep crammed on the floor or on the seats. For washing we rely on the "bucket system" in small cabin rooms that get constantly flooded with (rather filthy) water. Surely I'm a white fly on board and everyone wants to speak with me. In spite of this and the breathtaking view of the sunset, I get on with my work...

Cory Solovewicz receives more unwanted emails than you. Seriously—it’s a lot more. Since December 2024, one of the domains at which the #security researcher receives #email has registered 401,796 messages—by his calculations that’s an average of 699.99 pings per day.
This deluge isn’t the regular flood of spam, newsletters, and unwanted deals that fill many people’s inboxes. Instead, companies and other organizations are inadvertently sending Solovewicz other people’s private information and company #secrets. Over the last few years, he’s received injury reports from a city government, confirmation of people’s pizza orders, and account setup emails from a school platform. “I get service orders for people that need repairs. I get lots of test platform credentials,” says Solovewicz, a security researcher and consultant.

There are no bus schedules, no tickets, and buses depart when they are full. I am getting to the main departing point in Madang when it's already late in the morning - but I really wanted to talk about locals about coastal erosion in the city. Fortunately, I still find a place for me on probably the last bus, and two seats are allocated to my bulky luggage. Off we go for the last stop in Papua island before hopefully embarking for the Bismarck archipelago.

Day 65: After a smooth 9-hour boat ride over the Sepik river, we made it to Sisimagung, Northern Papua. This is the 65th day of my no-fly trip from Italy to the Solomon islands. The coastline is beautiful, what you could describe as paradise on earth. Coconut trees gently bend over sandy beaches and the sound of the waves quietly resound among villagers busy with their fishing nets or cooking earths. It is then all the more upsetting that this beauty is slowly but incessantly being destroyed by sea level rise. This is what Jeffery Bae, a local resident, tells me in the few hours I have here before getting on a truck to Madang. It seems impossible that a sea so quiet can become the agent of destruction, but this is precisely what happens, as Jeffery shows traditional houses made with sago and bamboo leaves having been dismantled by the latest storm surges.
"We will have to relocate", tells me Jeffrey with an empty, "but we don't know where, because right behind our village there's a swamp".
Once more, I hear the same story: no help from national, regional governments, and neither from international organisations.

It's a very short boat ride the one from Singapore to Batam, Indonesia. But it's really a springboard from the globalised, motorised, and carbonised part of the world to the Global South. I have already travelled about 14,000km to come here without taking airplanes and I truly savour the crossing of this physical but most importantly cultural boundary between North and South.
The taxi that takes me to the embarkment to Jakarta travels through bumpy roads, which turn into dirt road at the very access to the Pelni (the Indonesian national ferry company) departure point. I am torn between accepting the offer of porters to carry my luggage or carrying it myself. It's little money for me and a lot of money for them. I opt for carrying it myself, as I am short of Indonesian cash and I perceive porters' requests as bordering extortion. I travel 3rd class, because I want to stay as close as possible to the travelling conditions of most travellers. I get a bed in one of the many sleeping halls provided on the ship. A mattress is provided, although it's better not to think of the last time it has been washed. I must say sleeping is hard. People talk loudly throughout the night and kids cry loud. However, I am so tired that I sleep tight. As usual, I am woken up at 4am by the call to the Islamic morning prayer.

For once, an #AI issue where I *can* speak with a certain amount of authority.
I've been hearing "expert systems outperform human #diagnostics, so pretty soon human #physicians will be obsolete" for a few years now. It's closely akin to "#airliners fly themselves these days, so what do we need #pilots for?" In both cases, people are paying attention to the best-case scenario with no understanding of the *enormous* number of how many and various the worse cases really are.
This is complicated by the fact that in most of #medicine (although not necessarily #emergency medicine, the author's specialty) and in nearly all of air travel, the best case is also the normal case. Most of the time, whatever is wrong with you can be diagnosed and treated. Almost all the time, when you get on a plane, you'll walk off at the other end of the trip as healthy as when you boarded. It's reasonable to expect those outcomes.
Not-best and not-normal cases add up really fast.
Without any false modesty whatsoever: as a #medic, I learned a truly impressive degree of clinical judgement. From the first moment I saw a patient, I had a pretty good idea of #diagnosis, #treatment, and #prognosis. (Sadly, if my initial call was "this one's not going to make it," I was almost always right. The exceptions kept me going.) I learned from the best—one of my mentors had learned *his* trade in rural Guatemala, where resources were terribly sparse and human judgement was the only line between life and death. He held back death for decades, and it came for him far too early. Gene Gibbs, RIP.
I can't code that. Neither can anyone else, and if they tell you they can, they're lying.
As a #researcher, I've done a fair amount of work in #clinical #decision #support (#CDS). The idea is simple, and valid: no human, or team of humans, can remember everything they need to know. There's simply too much knowledge for the brain to hold and recall on demand. Subtle relationships exist between disparate types of data that *nobody* knows, until we tease out the numbers. We're doing this, right now. It is saving lives and relieving suffering, right now.
The key word there is "support." Humans still absolutely, positively, 100% need to be in the loop.
Maybe that will change, someday. I'm not saying it's impossible, for two reasons. First, any time anyone says "computers will never be able to ___" they're usually proven wrong. Second, I don't want to limit my and my colleagues' imaginations. We need to stay focused, but it is a *good thing* for our reach to slightly exceed our grasp. That's how #science happens!
Just not this day, and not for many days to come. Right now, we need to keep muddling along. There's not much more human than that.
