Is it some kind of systemic failure or more prosaic bribery &/or corruption? What are the remedies or penalties? Did someone/s benefit in some way or was there a collective fantasy? Did a spreadsheet go rogue or were numbers missing or not known or not included in cost-benefit calculations? Could lead to potential for beneficial learning & preventive future actions.
