TITLE: Innovative virtual reality treadmill therapy offers relief for treatment-resistant PTSD

    URL: psypost.org/innovative-virtual

A study involving Canadian military veterans, public safety personnel, and healthcare workers showed that their PTSD, depression, and anxiety symptoms decreased after 10 to 14 weeks of 3MDR therapy. Their difficulties with life functioning were also reduced. The improvements were still present 12 months after the treatments ended. The paper was published in Brain and Behavior.

Multimodal Motion-Assisted Memory Desensitization and Reconsolidation (3MDR) is an innovative psychotherapy developed for people suffering from post-traumatic stress disorder (PTSD), particularly those who have not responded sufficiently to conventional trauma-focused treatments. It combines established principles of exposure therapy (a psychological treatment that helps people safely confront traumatic memories) and eye movement desensitization and reprocessing (EMDR) with walking and an immersive virtual environment.

During treatment, patients walk on a treadmill toward personally selected images related to their traumatic experiences, while music and other cues are used to help evoke relevant memories and emotions. With the therapistโ€™s guidance, patients describe the memories, thoughts, feelings, and bodily reactions that emerge rather than avoiding distressing traumatic material. An attention task involving eye movements is then used while the traumatic memory is active, with the aim of facilitating further processing and reducing its emotional intensity. The combination of movement, immersive stimulation, exposure, and memory-processing techniques is intended to make it easier for patients to approach memories that they might otherwise strongly avoid.

3MDR was initially developed for military personnel and veterans with combat-related and treatment-resistant PTSD (defined as having tried two or more previous therapies without success) but has subsequently been studied in other groups exposed to severe trauma. Overall, 3MDR represents a promising new approach for treating PTSD, especially in cases where avoidance or limited response to conventional therapy makes treatment particularly difficult.

Study co-first authors Matthew Robert Graham Brown and Phillip R. Sevigny, along with their colleagues, conducted a study in which they examined the effectiveness of 3MDR in Canadian military veterans. They also wanted to test it with public safety personnel (police, firefighters, paramedics, etc.) and frontline healthcare workers. Their general hypothesis was that 3MDR treatment would lead to improvements in mental health symptoms of participants and that the improvements would still be present a year after treatment.

Study participants were initially 56 Canadian military members and Veterans, public safety personnel, and healthcare workers. However, eight participants were excluded from the analysis for various reasons, making the total number of study participants 48. Twenty-four participants were women. Their average age was 45.4 years. Seventeen participants were Canadian military veterans or active members of the military, 21 were public safety personnel, and 21 were healthcare workers. All of them exhibited clinically relevant symptoms of PTSD. Twenty-two of them met criteria for treatment-resistant PTSD, and this included all 17 military members and Veterans.

Participants received 3MDR therapy at the Edmonton Operational Stress Injury Clinic (EOSIC), which partnered with the research team for this study, or at one of three clinical research sites: University of Alberta main campus in Edmonton, Alberta; Alberta Hospital Edmonton; or University of Alberta space rented at the University of Calgaryโ€™s downtown campus in Calgary, Alberta. The 3MDR treatment lasted 10 to 14 weeks.

At the start of the study and at the end of the treatment program, participants completed the CAPS-5 clinical interview (an assessment of PTSD symptoms). A smaller group of 30 participants who were available for the follow-up also completed it 3, 6, and 12 months after the end of the treatment. At the start of the study and after specific sessions during the treatment, participants also completed assessments of depression, anxiety, and difficulties with life functioning. Participants completed an assessment of resilience before the 3MDR treatment and after it was complete.

Results showed that participantsโ€™ PTSD symptoms, depression, anxiety, and difficulties with life functioning improved after the 3MDR treatment. These improvements were still present 12 months after the treatment ended. Resilience also showed improvement from before to after the treatment.

โ€œThese results support the growing body of literature illustrating 3MDR as an effective treatment for military-related PTSD as well as PTSD-related mental health conditions in public safety personnel and healthcare workers,โ€ the study authors concluded.

The study sheds light on the effectiveness of 3MDR therapy as a treatment for PTSD. However, the study did not include a control group, although the study authors anticipated a natural waitlist control group to form due to expected scheduling delays, which ultimately did not occur. Because of this, it remains unclear how much of the reported improvements are a consequence of the treatment and how much can be attributed to natural reductions in symptoms over time or factors not related to the 3MDR treatment.

The paper, โ€œ3MDR Therapy Reduces Symptoms of PTSD and Related Conditions in Canadian Military Members and Veterans, Public Safety Personnel, and Clinical Personnel,โ€ was authored by Matthew Robert Graham Brown, Phillip R. Sevigny, Katherine S. Bright, Helen Chau, Patricia Chai, Andrew J. Greenshaw, Lisa Burback, Olga Winkler, Chelsea Jones, and Suzette Brรฉmault-Phillips.

  23. Came down with a bug last week and only started to feel human yesterday, so I thought a run would help shift the last of it. Did my longest run so far, I literally ran until my legs turned to jelly and I went dizzy. I ended up lay on the floor trying to overcome a panic attack as I couldnโ€™t breathe or see properly but despite that a coastal run blew away the cobwebs and made me feel like I had achieved something.
It was a quiet run and there was hardly anyone about, which is very much my kind of run. Pushing myself and getting out of my comfort zone is getting out there for a run or hike but right now it has to be somewhere quiet with very few people about as groups of strangers still sends my mental health spiralling and causes my PTSD to go worse and also triggers more flashbacks than I usually deal with, so itโ€™s too much for me right now. Iโ€™m great with groups of people I know and feel safe with Iโ€™m just not there yet to push myself further but I will get there. Planning ahead really helps but I still canโ€™t plan for everything and some things slip through the cracks. This time it was becoming dehydrated as I donโ€™t have a running vest to carry fluids while I run yet. This is something Iโ€™m going to have to move up my priority list if I want to do another long run, so I will get saving for one.
Proud of myself for continuing to push my boundaries by trying somewhere new to run and giving it everything I had in the tank. My body and mind will get stronger and I know it is going to be a long journey. I am working very hard on accepting that recovery is going to take time but as long as I keep trying I will eventually get there. #mentalhealth #mentalhealthrecovery #mentalhealthawareness #ptsd #ptsdrecovery #complextrauma #running #runningmotivation #coastalrun #runningrecovery #freshair #pushyourlimits #keepgoing #tryingmybest

  24. After a week of not running I got myself back out there. Super proud of myself because itโ€™s taken me days to push myself back out the door.
I struggle massively to get out and exercise I have to have loads of things in place and itโ€™s a whole thing just to force myself outside. The promise I made to myself that I will push myself this year to improve my mental health is thankfully still the loudest voice. The self doubt, anxiety, darkness and trauma are still extremely loud and I battle with poor mental health constantly but the important thing is that Iโ€™m still fighting, fighting for myself. None of this is easy at times it feels impossible but I donโ€™t allow myself to give up on myself. If you are struggling with your mental health remember you are worth fighting for and fight for yourself, just a little win makes all the difference. Youโ€™ve got this and you are so worth it, keep going and keep fighting for you ๐Ÿ’ช๐Ÿผ๐Ÿƒ๐Ÿฝโ€โ™€๏ธ๐Ÿ‘Œ๐Ÿผ. #mentalhealth #mentalhealthmatters #mentalhealthawareness #mentalhealthrecovery #ptsd #ptsdrecovery #ptsdrecoveryjourney #complextrauma #running #runningmotivation #thisgirlcan #thisgirlcanrun #positivity #keepgoing #youareworthitโค๏ธ #youmatter #fightforyou

  25. Nutrition has always been an issue for me, carbs and sugar are very much my downfall. I have now committed myself to my mental health and fitness journey and a huge positive contributing factor to that is eating healthier and ensuring my body now gets the nutrition it needs. My lifestyle is changing and I need better eating and drinking habits for my body to heal and be able to progress.
The 1st change I have made is drinking water, I barely drink water which I know is awful but this journey is about honesty and honestly some days I donโ€™t even drink a glass of water. So that was the 1st thing that needed to change, I have frozen berries and a scoop of collagen in my water bottle. I then use the berries to get a portion of fruit down me, Iโ€™m really not a lover of fruit so the past two mornings Iโ€™ve added it to a protein yoghurt for my breakfast. Any tips on a healthy way to eat my portion of berries are very welcome. I then refill my water bottle and drink that.
    Iโ€™m making gradual changes as I get easily overwhelmed and I donโ€™t want to affect my mental health or have my brain process these changes as anything but positive. Iโ€™m happy with the start Iโ€™ve made, itโ€™s been a long time coming. #mentalhealth #mentalhealthmatters #mentalhealthawareness #mentalhealthrecovery #ptsd #ptsdrecovery #healthyeating #healthyfood #healthyliving #healthylifestyle #healthybodyhealthymind #nutrition #recovery #recoveryjourney #exercise #runningtherapy

  26. As I am still a non smoker Iโ€™ve decided to use some of the money saved on myself. Itโ€™s so easy to let the โ€œextraโ€ money get eaten up by bills etc because letโ€™s be honest when you are on a tight budget there are always essentials to spend our money on, especially with rising prices.
When I smoked I used rolling tobacco and smoked very little so my โ€œextraโ€ isnโ€™t loads but itโ€™s still more. Iโ€™ve thought a lot about what I can do for myself and Iโ€™ve chosen something fairly simple. I need to drink way more water than I do and as Iโ€™m not far off 46 I need to start looking after my body more before menopause hits. Iโ€™m aware that when menopause does hit my hair, skin, nails and joints will be affected so Iโ€™m going to try a daily dose of collagen to see if I notice a difference. I also barely eat fruit as Iโ€™m not too keen on it so Iโ€™m also going to add some frozen berries to my water, that I will eat once Iโ€™m done with my water.
I am noticing that the big boost in exercise whilst great for my mental health, those natural hormone boosts that come with exercise are super helpful for someone like me with awful mental health but no medications have worked for me so Iโ€™m reliant on exercise and also Iโ€™m now looking at diet and supplements to also possibly help, my body is struggling to keep up and recover.
Iโ€™m hoping by starting to make little changes to my diet and other things that my recovery will start to improve and my body will start to keep up. If nothing else Iโ€™m getting much needed water into my body. I will update when I notice a difference. #mentalwellbeing #mentalhealthawareness #mentalhealthmatters #mentalhealthrecovery #ptsd #ptsdrecovery #exercise #activelifestyle #healthcare #stopsmoking #stopsmokingstartliving #fruit #water #collagen #takecareofyourmentalhealt #littlechanges #littlechangesbigresults #takecareofyourself #takecareofyourtemple #takecareofyourmachine

  27. Needed to escape from my head and get a win. So I did my 2nd hike yesterday up Pendle Hill over 1,800ft it was stunningly beautiful, challenging and peaceful in equal measure. The silence was something else, there was barely a soul about and the quiet and stunning views were something else. My mind completely emptied surrounded by the beauty and peace. I donโ€™t know if Iโ€™m just picking the right time or places or both but my 2 hikes have been so quiet and solitary, itโ€™s been perfect. I achieved a higher elevation so I got my win and absorbed so much beauty it made my heart beyond full. Got quite emotional when I got to the top. #mentalhealth #mentalhealthawareness #mentalhealthmatters #mentalhealthrecovery #ptsd #ptsdrecovery #nature #naturelover #naturalbeauty #pendlehill #hike #hikelife #hikevibes #hikeingadventures

  28. Two months of not smoking, thatโ€™s huge for me. Still very much taking it a day at a time and still using nicotine mints and gum but even that is getting gradually less. Been trying to work on running faster so Iโ€™ve been doing shorter faster runs. Canโ€™t say Iโ€™m getting the same benefit from it mental health wise as it was supposed to be about escaping into my runs to switch off Iโ€™m actually not sure how it ended up the way it has. I do want to beat my 5k PB but once I achieve that I can go back to enjoying my runs again. Iโ€™m honestly unsure that itโ€™s the right move for me as my mental health has took a nose dive, the 2nd morning was brutal it took about an hour to get out the door and I had sat crying for 20 mins then cried during my run. It then didnโ€™t help getting laughed at by some girl running the opposite way with her mate, itโ€™s the 2nd time this has happened since I started back on the 1st January.
So to all you fit, fast, skinny birds good for you that you look great and are fab runners. Not all of us are like you, yes Iโ€™m fat and awkward but Iโ€™m having a go and working ridiculously hard to get out the door to run and improve my physical and mental health. So perhaps think twice before giving a snide look and laughing, try and put yourself in my shoes. To the beautiful humans that smile and encourage thank you so very much these small interactions make difficult days brighter.
    Still going and still trying to improve my life and mental health ๐Ÿ’ช๐Ÿผ๐Ÿƒ๐Ÿฝโ€โ™€๏ธ. #mentalhealth #mentalhealthmatters #mentalhealthawareness #mentalhealthrecovery #ptsd #ptsdawareness #ptsdrecovery #running #runningmotivation #runningtherapy #bekind #thisgirlcanrun #keepgoing #staystrong #positivevibes

  29. Even with the exercise my mental health is still a major battleground. The endorphines are a huge help as none of the many many medications Iโ€™ve tried have worked for me, they actually made me feel way worse.
I still have to plan and put things in place so that I can force myself out the door and even then it can all go to pot. So Iโ€™m having to respect my limitations whilst trying to push myself as hard as I am able. I was supposed to run yesterday but it took my two hours to force myself out of bed and I couldnโ€™t force myself out that door no matter how hard I tried it was all too much. I though today would be the same as I couldnโ€™t get myself out of my bed and face the world, it took an hour to get me out of the door and I almost turned around and went back home a few times. I truly pushed myself to my limit today mentally and physically every step was hard fought for so Iโ€™m really proud to of ran 11.5k running until my legs could go no further and I ran my fastest 10k.
    Fighting against PTSD and complex trauma is a beast, one that frequently brings me to my knees but one I keep fighting anyway. #mentalhealth #mentalhealthawareness #mentalhealthmatters #ptsd #ptsdawareness #ptsdrecovery #complextrauma #complextraumarecovery #running #runningmotivation #runningtherapy #10k #thisgirlisonfire #thisgirlcan #thisgirlcanrun

  30. In the latest instalment of Emmaโ€™s midlife crisis she goes on her 1st hike. Encounters some serious beauty and surrounds herself in nature. At times itโ€™s that quiet and there are no people around that she feels like the only soul on the planet.
Rivington pike loop is a challenging hike, the elevation gains at times had me hearing my heart in my ears and was really out of breath. The steep walk down was a huge challenge for me as I hate heights at one time I wouldnโ€™t of had the courage to take such a steep route down so Iโ€™m really proud of myself for that. Some very boggy areas on the moors to navigate, again a new experience for me. 31. Had a brutal night filled with really traumatic night terrors so all I wanted to do was wrap myself in my blanket and wallow in the darkness. I pushed myself really hard not to give in and help myself by getting out the door. My head gave me every reason not to go out and run but I fought back really fucking hard got myself out the door and get some exercise, fresh air and switch off the darkness and pain.
Iโ€™m really proud of my achievements today as they were fought for, trying to recover from complex trauma and PTSD is a tough and long road but on Iโ€™m determined to keep walking (or running). I got up and I put my make up on (a mask of make up is essential for me when I feel this crap about myself or I wonโ€™t get out the door) then went to the river for a morning run. It was a challenging run my brain telling me Iโ€™m not good enough in so many ways but I needed the win so I took control and got myself the win. I know this doesnโ€™t fix anything but I needed to remind myself that I am a strong and powerful woman that can fight back, even against my own poor mental health. #mentalhealth #mentalhealthmatters #mentalhealthawareness #ptsd #ptsdawareness #ptsdrecovery #running #runningtherapy #riverrun #win #postivevibes #hope
