Therapy has imitators: platforms that dispatch counselors like takeout, TikTok influencers handing out diagnoses, coaching packages and chatbots rebranded as care. They borrow the language of therapy and the promise of relief. They leave out what makes it work.
What Sets Psychotherapy Apart
Psychotherapy rests on a relationship between therapist and client, the safety to question your assumptions and face your blind spots, and the privacy to do that work unobserved. The research Duane surveys keeps returning to this core: outcomes depend less on the particular technique than on the quality of that relationship. An emerging neuroscience framework he describes, predictive processing, offers a promising account of why, tracing how the brain updates its model of the world when a person feels safe enough to do so. It is not settled science, but it points where careful clinicians have long pointed.
