Perhaps it does make sense to make that extra stage theme so emotional, if you consider the context at the time that Touhou 5 was supposed to be the final game of the series. Which also explains why the staff roll theme after beating the Extra stage is so emotional and sad too.
Mystic Square was supposed to be the last hurrah, the last loud bang that ZUN made for the series.
But we obviously know what happened next, and thankfully we got Embodiment of Scarlet Devil instead of letting the series be yet another obscure PC-98 video game series forever to be forgotten in time. And it is thanks to EoSD and later games that the Western fandom that got established afterwards will get to know about the PC-98 through the first five games, including Mystic Square!

Anyway Alice in Wonderland is not my #1 favorite (that goes to SoEW's Extra Love, I just like how upbeat it is), but it definitely is one of my favorites. It's just so emotional, calm, and deeply designed in composition. It's so unlike the previous Extras, and perhaps even the Extras in Windows (though I admittedly haven't heard all of them yet).
