9 patients in our #OPAT clinic today. That's 9 people who would otherwise have been in hospital for Christmas.

Everyone's getting a cracker. I haven't won a pull yet.

Sometimes patients bring gifts on their last day in the #OPAT clinic. A card, biscuits, something like that.

One patient made us pens.

Five years ago today.... the empty OPAT bay waiting for our very first patient to arrive.

Nearly 11,000 infusions later, and two changes of location, I'm feeling pretty darned pleased about what we've achieved.

To celebrate, I'll drop some #OPAT info today, in between work calls.

Firstly, OPAT is a great example of patient centred care. Patients almost universally prefer to be in their own homes (no late night emergencies, no unfamiliar beds, no hospital food).

It helps with rehabilitation & avoids loss of activity and confidence that goes with admission for prolonged periods (some of our patients attend daily for several weeks). Working patients can often fit appointments around their work schedule.

Well, we just passed a significant milestone in our #OPAT clinic.

(OPAT = Outpatient Parenteral Antimicrobial Therapy. It's a 7 day service that allows patients who need intravenous antibiotics to be at home rather than in hospital and to attend clinic once daily for treatment. Patients prefer it because they get to be at home, and we can free up acute beds).

We started in November 2019. Remember, when we had just heard about Wuhan...?

Today, we have delivered infusion number 10,000!

Hi! I'm David, and I work in #Scotland (Kingdom of Fife) as an #NHS doctor specialising in #Microbiology and Infectious Diseases.

I have an interest in antimicrobial resistance (#AMR) and antibiotic stewardship, and I run our local #OPAT service, where we deliver daily IV antibiotics to patients otherwise well enough to be at home.

Just had our busiest ever week in the outpatient antibiotic clinic (OPAT).

73 treatments delivered, saving an equivalent number of bed days.

That's an average of over 10 patients per day who can be at home with their families, even go to work in some cases, but more generally just get well in their own environment.

It's equivalent to half-emptying a ward for the week.

Plus, our service is about £500 / day cheaper than inpatient care, so we just saved the NHS about £36,000.

It is great, but it needs to be properly managed and have great communication (& people) to avoid anyone feeling like they are practicing less safe medicine. As the paper says, safety-nets are key.
