Amazing that all of Manawanui got out alive!
Same thing happened on USS Fitzgerald and again on USS John McCain and the death toll was over twenty sailors in total.

A Naval Court of Inquiry has found that the good ship Manawanui grounded in Samoa because of human error.
The crew didn't realise the ship was on autopilot (doh!) so the vessel ignored their efforts to change direction and sailed happily towards land until it grounded.
It'll be back to Nav-101 for somebody.

ℹ The Royal #NewZealand #Navy has lost its first ship to the sea since World War Two, after one of its vessels ran aground off the coast of #Samoa.
HMNZS #Manawanui, a specialist diving and ocean imaging ship, came into trouble about one nautical mile from the island of #Upolu on Saturday night local time, while conducting a survey of a reef.
It later caught fire before capsizing.
All 75 people on board were evacuated onto lifeboats and rescued early on Sunday
