Perhaps it does make sense to make that extra stage theme so emotional, if you consider the context at the time that Touhou 5 was supposed to be the final game of the series. Which also explains why the staff roll theme after beating the Extra stage is so emotional and sad too.
Mystic Square was supposed to be the last hurrah, the last loud bang that ZUN made for the series.
But we obviously know what happened next, and thankfully we got Embodiment of Scarlet Devil instead of letting the series be yet another obscure PC-98 video game series forever to be forgotten in time. And it is thanks to EoSD and later games that the Western fandom that got established afterwards will get to know about the PC-98 through the first five games, including Mystic Square!

Did you know that #Touhou 5: Mystic Square was the first game in the series to introduce more than one boss being fought at the same time in a fourth stage? This will not be repeated until the 7th game (2nd game for the Windows era) Perfect Cherry Blossom, where you fight three bosses at once.
