  1. I Have Kallmann Syndrome. I Am Not Transgender.

Nicky Chaleunphone is a DSD person with Kallmann syndrome, not transgender. This page is the public record for anyone who searched that name and that accusation.

After photo shoots and public posts, the same assumption keeps landing in comments and search boxes: that I must be transgender. I am not. I have Kallmann syndrome, a congenital endocrine condition that affects puberty and the sense of smell. I describe myself as a person with a difference of sex development who lives in the body that condition produced. That is a medical and developmental fact. It is not a gender identity I adopted later so I could wear dresses or do modeling work.

I am writing this so the record is in my words. Years ago, someone tried to shut me up by calling me transgender. The tactic is old and simple. Take a body that does not follow the usual script, relabel the medical difference as an identity claim, then treat the identity claim as something you can dismiss. I will not keep leaving that unchallenged while new people find my work and repeat the same error.

    What Kallmann Syndrome Actually Is

Kallmann syndrome is a form of congenital hypogonadotropic hypogonadism paired with a reduced or absent sense of smell. According to the 2015 European consensus statement published in Nature Reviews Endocrinology, congenital hypogonadotropic hypogonadism is caused by deficient production, secretion, or action of gonadotropin-releasing hormone, the master signal of the reproductive axis. When that condition is joined to anosmia or hyposmia—found in about half of patients—it is called Kallmann syndrome, and it results from incomplete embryonic migration of GnRH-synthesizing neurons.1

A later comprehensive review in Endocrine Reviews describes the same mechanism in clinical language. Pulsatile GnRH secretion is what starts and maintains human reproductive capacity. When that pulse generator never comes online in the usual way, puberty is delayed or absent and infertility follows unless the axis is treated. Kallmann syndrome is the form of that condition that also includes an impaired sense of smell, along with other possible associated anomalies.2

The StatPearls clinical review puts the timeline where patients actually meet it: diagnosis often arrives when puberty fails to begin. A deficiency of GnRH leads to low gonadotropins and low sex steroids, so secondary sex characteristics do not develop on the usual timetable. The olfactory finding is present from early life, even if nobody tested it until a doctor asked.3 MedlinePlus Genetics, maintained by the National Library of Medicine, lists the features families and clinicians actually see: delayed or absent puberty; micropenis or undescended testes in some newborns assigned male; missing secondary sex characteristics; infertility without specialized treatment; and, in some people, additional findings such as a kidney that never formed, hearing differences, cleft lip or palate, or dental anomalies.4

That list is development. It is an endocrine circuit that did not complete its migration and never delivered a normal puberty signal. It is not a social identity.

I am not going to turn this post into my full chart. You do not need every lab value to understand the point. The diagnosis describes how the reproductive axis and the olfactory system formed. Hormone replacement in this setting is treatment for hypogonadism—protection of bone, induction of secondary characteristics that never arrived on their own, metabolic and sexual health—not a protocol for rewriting sex as an identity project.5 Patients typically need lifelong treatment, though a minority later show some spontaneous recovery of reproductive function.6

What “DSD” Means in Medicine—and What It Does Not Mean

In 2006, an international consensus conference organized by the Lawson Wilkins Pediatric Endocrine Society and the European Society for Paediatric Endocrinology replaced older, often pejorative terms with “disorders of sex development.” The definition they published is specific: congenital conditions in which development of chromosomal, gonadal, or anatomic sex is atypical.7 The companion paper in Archives of Disease in Childhood uses the same definition and the same proposed lexicon.8

Many clinicians and advocates now say “differences of sex development” for that same group of congenital conditions. A 2025 primer in Nature Reviews Disease Primers still starts from the Chicago framework: sex chromosome DSD, 46,XY DSD, and 46,XX DSD, with subclassifications according to gonadal determination and hormone-dependent differentiation of internal and external sex organs.9

Kallmann syndrome is classified in endocrinology as congenital hypogonadotropic hypogonadism and Kallmann syndrome, not as every textbook’s first example of genital-ambiguity DSD. I still use DSD language for myself because the condition is congenital, it altered the hormonal path that produces adult sex characteristics, and it has left me with a body that does not match the binary script people expect when they look at legal sex, clothing, or photographs. That is a first-person description of living with the condition. It is not a claim that every CHH paper uses the Chicago heading.

Advocacy and clinical education groups that actually work in this field have already answered the question this post exists to answer. Accord Alliance, which focuses on DSD clinical service delivery, states the distinction directly: transgender refers to a situation in which a person feels that the gender assigned at birth was not the right one; DSD is about physical sex development—how a person’s body formed—not about gender identity.10 interACT: Advocates for Intersex Youth includes Kallmann syndrome in its variations glossary and describes it as a form of hypogonadism that can present with micropenis and undescended testes in people with XY chromosomes and with delayed or absent pubertal changes unless hormone therapy is given.11 That is a body-development entry, not a gender-identity entry.

Sex Development and Gender Identity Are Not the Same Variable

Sex in humans is a biological classification grounded in chromosomes, gonads, anatomy, and reproductive physiology. Gender identity is a person’s subjective sense of being male, female, or something else. The 2025 DSD primer is careful to treat those as distinct concepts: gender identity develops over time and is not simply a direct function of genes or prenatal hormone exposure.12 You can have an atypical developmental path and still not have a transgender identity. You can have a typical developmental path and still have one. Mixing the two categories is how people erase patients like me.

Large outcome research on DSD populations treats gender dysphoria and later gender change as possible findings to screen for, not as the definition of the medical category. In the dsd-LIFE study published in the Journal of Sexual Medicine in 2018, gender changes were reported by 5 percent of participants. Only in 1 percent—3 percent if Klinefelter and Turner syndromes are excluded—did the change take place after puberty and appear likely to have been initiated by the patient. Four percent reported some form of gender variance. The authors’ clinical point was that clinicians should be aware the possibility exists, not that DSD means transgender.13

Most people with a DSD are not transgender. I am one of them.

I do not identify as a man or as a woman in the way online debates use those words. I also do not identify as transgender. I live in a Kallmann body. I present in the clothes that work for that body. I do not owe anyone a third label so their spreadsheet balances. “My body, my choice” is not a slogan I borrowed. It is how I have had to live.

    Clothes Are Not a Diagnosis

I wear dresses, onesies, maxi skirts, and the outfits I wear to shoots because that is how I am comfortable and how I work. Modeling, boudoir, shibari, castle locations—that is presentation and labor. It is not a coming-out statement. People see the pictures and reach for the only public story they have been handed: if the person is not doing masculinity the expected way, the person must be trans.

That story is a category error. Clothing does not rewrite embryology. A photograph does not change GnRH neuron migration. Hormone treatment for hypogonadism is replacement for an axis that never switched on in the usual way. The European consensus and the later Endocrine Reviews synthesis are about diagnosis, inducing puberty, bone and metabolic health, and fertility options. They are not identity manuals.14

I can dress how I dress without converting a congenital endocrine diagnosis into a transition narrative.

    Why Search Results Matter

I am publishing this in the form of a long, dated, first-person page on a site I own because that is how search works. A stranger who types my name next to the word “transgender” should hit my sentence, not someone else’s label. The opening of this post repeats the name, the diagnosis, and the denial on purpose. That is not branding. That is the record.

I already talk about Kallmann syndrome and DSD on the podcast, in modeling bios, and in older interviews. This page exists so those statements have a single canonical address I can point to from every profile I control. I am not interested in a sixteen-year-old argument with a dead blog. I am interested in the pattern. Calling me transgender has been used as a way to change the subject: stop talking about the medical condition, stop talking about the work, start defending or denying an identity I never claimed. Once the label sticks, the speaker gets to treat the whole file as ideology.

That is sloppy medicine and sloppy rhetoric. Kallmann syndrome has a definition, a mechanism, and a treatment literature.15 Transgender identity has a different definition.16 Using the second to overwrite the first is not inclusion. It is a refusal to look at the actual diagnosis.

I am not saying DSD and transgender identity never coexist in anyone. The dsd-LIFE numbers show a small minority.17 Coexistence in some people is not a license to apply the label to this person.

    What I Am Asking For

Read the condition, not the costume.

If you want a medical source, start with the European consensus on congenital hypogonadotropic hypogonadism or the 2019 clinical-management review in Endocrine Reviews. If you want the DSD terminology, read the 2006 Chicago consensus. If you want the short answer to whether DSD is the same as transgender, Accord Alliance already published it: no.

Questions about the condition, asked in good faith, are fine. Lectures that I am “really” trans, that I am erasing anyone, or that my presentation overrides the diagnosis are not. I will not argue endocrinology in a comment section. I will not perform a gender identity I do not have so that someone else’s categories stay tidy.

I have Kallmann syndrome. I am a DSD person. I am not transgender. Attempts to relabel me are not curiosity. They are a way to change the subject. That accusation has been used to silence me. It does not get to do that again.

  3. Day 6 of the LGBTQ alphabet series: I is for Intersex.

There is a lot of confusion about the term Intersex and Intersex people. In short, Intersex refers to people who are born with sex characteristics (chromosomes, sex organs, etc.) that do not fit the traditional binary, male/female, norms.

To be honest, I am still learning about Intersex and Intersex people, so if you are reading this article and want to learn more then you may want to check out the longer version of this article on Substack.

It should be noted that while older medical references often used the term 'hermaphrodite,' today that term is considered scientifically inaccurate and deeply stigmatizing. The correct, respectful, and universally accepted term is intersex.
