  1. Translating artificial intelligence into clinical practice for gastrointestinal endoscopy: current applications and future perspectives

Artificial intelligence (AI) has emerged as a transformative tool in gastrointestinal (GI) endoscopy, addressing challenges in detection, diagnosis, and decision-making. In upper GI endoscopy, AI supports blind spot monitoring, Helicobacter pylori diagnosis, and the identification of premalignant and malignant lesions, with high accuracy and reduced miss rates. In lower GI endoscopy, computer-aided detection improves adenoma detection, whereas computer-aided diagnosis supports “resect-and-discard” and “diagnose-and-leave” strategies. However, real-world benefits remain modest, with concerns regarding overdetection and variable performance across lesion types and colon segments. In inflammatory bowel disease, AI standardizes endoscopic and histologic scoring, reduces interobserver variability, and accelerates capsule endoscopy interpretation, including high diagnostic accuracy for Crohn’s disease. Pancreatobiliary applications, including endoscopic ultrasound, endoscopic retrograde cholangiopancreatography, and cholangioscopy, demonstrate strong performance in differentiating pancreatic masses and biliary strictures and in predicting postprocedural complications. Despite expert-level performance across multiple domains, most studies remain single-center or retrospective, and explainability, workflow integration, medicolegal responsibility, and cost-effectiveness continue to limit adoption. Emerging solutions, including explainable AI and AI-generated common data model-compatible reports, may bridge these gaps. With rigorous multicenter validation and real-world implementation, AI can evolve from an experimental adjunct into a core component of routine endoscopic practice.

OK... it's the afternoon because an early morning #Endoscopy interfered with my normal schedule. Better late than never, as no one ever says about my posts.
On this day in 1866, Renegade Irish Fenians from the U.S. invade Fort Erie, Ontario. Spoiler alert, the fiendish Fenians were unsuccessful. On the same day, a mere 61 years later in 1927, the Peace Bridge opens between Buffalo, New York and Fort Erie, Ontario. Coincidence?

  15. 🔬 Biopsy smart, not routine.

Knowing when to biopsy—and when not to— can change diagnosis, management, and outcomes in upper GI endoscopy.

From Barrett’s protocols to linitis plastica sampling—precision matters.

  16. I had my endoscopy this morning.
It showed that the ulcer has healed, but there's a patch of Barrett's Oesophagus, so a couple of biopsies have been sent to the histology lab to see if they're important or interesting.
    The hiatus hernia hasn't changed.
    So that's pretty good news.

However, the sedation that didn't affect me last time has completely clobbered me.
There's bits of the procedure I can't remember, and I've been asleep pretty much since I had some food & drink when we got home.

I feel really rather spaced out!

  34. CW: More boring health updates

Now I've got the Famotidine I can definitely feel that my stomach is settling down.

The appointment for the urgent endoscopy has come through for Monday afternoon.

The blood test results from yesterday are all now in the NHS app, and all are within the reference ranges.

I'm no where near as sleepy as yesterday, thank goodness

  44. In October, I'm going to get spit-roasted. I'm going to get an upper endoscopy to see what's going on with my esophagus. I'm likely to be suffering from GERD.

I'm also going to get a colonoscopy as a routine cancer screening.

Both procedures will take place one after the other.

I've started a course of proton pump inhibitor for my GERD yesterday... and... strangely... I feel that my mood has improved. I did a search and found medical articles that talk about the relationship between GERD and depression and anxiety. The relationship apparently goes both ways.
