  1. No benefit of vagus nerve stimulation compared to a sham procedure on PVCs. Regression to the mean, or truly a placebo effect? #cardiosky #metasci www.jacc.org/doi/10.1016/...

  2. As ridiculous as CKD staging is, we have nothing on this thing of utter beauty

We are all stage A HF! Flozinate everyone. GDMT on the drinking water.

  3. Systemic vascular resistance is not a realiable measure of left ventriclar afterload. In this study, noradrenaline actually decreased left ventricular wall stress even though SVR increased. Higher heart rate and contractility leads to a smaller LV, lower wall stress. #emimcc #cccsky #cardiosky

  4. RCTs have shown no benefit from IABP in acute ischemic cardiogenic shock, an observatiobal trial isn’t even hypothesis generating. No grounds for new IABP trials in this population in my opinion. #cardiosky #cccsky #emimcc

  5. Observational studies suggests higher mortality but lower thrombosis risk with VKA vs DOACs in CTEPH. What do people do? Personally I want a trial before switching to DOACs as standard of care here #emimcc #cardiosky

  6. Bayesian reanalysis can be a great way to assess a contested trial, by showing how strong a prior you need to nullify or inverse the results. I don't understand why they did not include a pessimistic prior here though (even as I see a sceptical one more realistic). #emimcc #cardiosky #cccsky #stats

  7. Single arm pulmonary embolism intervention studies are useless for anything than assessing adverse events, perhaps even more so than the registry studies. Just look at the discrepancy in mortality between RCTs and observational data without inclusion criteria. #emimcc #cardiosky

  8. A diagnostic tool, that decreases all cause mortality in HFpEF? That’s fairly hard to believe. #cardiosky

  9. This should be the death of Impella outside of severe cardiogenic shock. High probability it increases mortality. #cardiosky #cccsky

  10. Convincing evidence that there is a benefit to CTO, but still a question if it is worth the resources and costs of these long procedures. Look forward to reading the full paper, not least for the bayesian analysis. Hard to interpret without knowing the priors. #cardiosky

  11. I really don't understand this trial. Why would you randomise STEMI patients to get Impella and wait for 30 minutes before revascularisation? Time is more important than wall tension I think. Amazed it didn't cause more harm really. #cardiosky www.jacc.org/doi/10.1016/...

  15. One thing that strikes me from these guidelines (which do have many good parts) is how many of the interventions have strong Ia recommendations based on purely observational evidence. Putting the cart before the horse #emimcc #cardiosky

  17. Forward ejection fraction (LVOT derived stroke volume divided by end diastolic volume) is an interesting concept in mitral regurgitation. Conceptually, it's what the EF would be if it wasn't for the preload reduction from the MI. pmc.ncbi.nlm.nih.gov/articles/PMC... #cardiosky #echosky

  19. One statistical concept that I wish was more widely appreciated especially among clinicians, is winners curse. In small trials, the observed effect size of a «positive» trial is more dependent on the trial design (size) than what the true effect size is. #stats #medsky #cardiosky
