    Situation at a glance

    Since the last Disease Outbreak News on this event was published on 23 August 2024, three additional countries and one territory (Ecuador, Guyana, Panama and Cayman Island) have reported confirmed Oropouche virus disease in the Region of the Americas in 2024. In addition, imported Oropouche cases have been reported from Canada, the United States of America and countries of the European Region. As of 25 November 2024, a total of 11 634 confirmed Oropouche cases, including two deaths, have been reported in the Region of the Americas, across ten countries and one territory: Bolivia (Plurinational State of), Brazil, Canada, Cayman Islands, Colombia, Cuba, Ecuador, Guyana, Panama, Peru and the United States of America. Based on available information, WHO assesses the overall public health risk posed by this virus to be high at the regional level and low at the global level. As the arbovirus season is starting in the region, WHO urges countries at risk to strengthen epidemiological and entomological surveillance and to reinforce preventive measures in the population. This is crucial due to the geographical expansion of the virus and the possible new vectors and transmission routes, including vertical transmission, that could affect both the general population and vulnerable groups, such as pregnant women, their fetuses, and newborns.

    Description of the situation

    Since the last Disease Outbreak News on this event was published on 23 August 2024, three additional countries and one territory (Ecuador, Guyana, and Panama) have reported confirmed Oropouche virus disease in the Region of the Americas. In addition, imported Oropouche cases were reported from the Cayman Islands, Canada, the United States of America and a few countries in the European Region.

    Between 1 January and 25 November 2024, 11 634 confirmed Oropouche cases, including two deaths, have been reported in the Region of the Americas: Bolivia (Plurinational State of) (356 cases), Brazil (9563 cases, including two deaths), Canada (two imported cases), Cayman Island (one imported case), Colombia (74 cases), Cuba (603 cases), Ecuador (two cases), Guyana (two cases), Panama (one case), Peru (936 cases), and the United States of America (94 imported cases). Additionally, imported Oropouche cases have been reported in countries in the European Region (30 cases) (1).

    Cases and consequences of vertical transmission of Oropouche virus infection have been reported in Brazil and Cuba. Brazil has confirmed three cases of vertical transmission (two cases of fetal death and one case of congenital anomaly) and reported that under investigation are 15 fetal deaths, five spontaneous miscarriages, and three cases of congenital anomalies.[1] Additionally, in September, Cuba confirmed a case of congenital anomaly with two further cases under investigation.

    Abstract

    Background: Usutu virus (Orthoflavivirus usutuense, USUV), a neurotropic arthropod-borne RNA virus of the family Flaviviridae, is a zoonotic virus that has spread throughout the European continent over the last three decades, since its emergence in Italy in 1996. However, no cases of USUV have been reported in Portugal so far.

    Material and methods: In the scope of an active surveillance program for Orthoflavivirus, we collected growing feather samples from 249 red-legged partridges (Alectoris rufa) hunted in southern Portugal during the 2021-2023 hunting seasons. Samples positive for USUV were subjected to whole genome sequencing and strain characterization.

    Results: Two partridges tested positive for USUV. Phylogenetic analyses of whole and partial genomes assigned the USUV strains to the African 3 lineage, specifically the African 3.1 sub-lineage.

    Conclusions: Our study confirms, for the first time, the circulation of USUV in wild birds in Portugal. Active surveillance of hunted partridges proved to be a useful, accessible, and cost-effective method for USUV monitoring, further supporting their value as effective sentinels for Orthoflavivirus surveillance. Given the ongoing circulation of USUV and the increasing risk of its spillover to other domestic and wild animals, and humans, additional efforts are needed to improve virus surveillance in Portugal from a One Health perspective.

    Summary
    Background
    Yezo virus (YEZV) is an emerging tick-borne pathogen, which was initially reported in Japan in 2021. Only one patient had been reported in China so far. We aimed to describe the epidemiological, clinical, and laboratory findings of a series of patients, and to characterise the viral genomes of YEZV.

    Methods
    In this active surveillance and genomic analysis, we conducted active surveillance at Mudanjiang Forestry Central Hospital, Heilongjiang Province of northeast China. Participants were eligible for inclusion if they sought medical care for a recent tick bite between May 1 and July 31, in 2022 and 2023, and between May 1 and July 10, in 2024. We collected sera from participants to detect YEZV infection by meta-transcriptomic sequencing, real-time RT-PCR, and indirect immunofluorescence assay. We isolated YEZV by cell culture and characterised the pathogen by morphological and phylogenetic analyses.

    Findings
    A series of 18 patients with YEZV infection (12 male and six female; median age 53 years, IQR 45–60) were identified among 988 participants. The patients presented with fever (18 patients, 100%), headache (ten patients, 56%), dizziness (nine patients, 50%), malaise (three patients, 17%), lumbago (three patients, 17%), and cough (three patients, 17%). Nine (50%) patients had rash around the tick bite site and four (22%) had lymphadenopathy. Nine (50%) patients had gastrointestinal symptoms, and five (28%) had neurological symptoms. We observed leukopenia in ten (63%) and thrombocytopenia in five (31%) of 16 assessed patients. Elevated hepatic transaminase concentrations were identified in 13 (72%) of all 18 patients, lactate dehydrogenase or α-hydroxybutyric dehydrogenase in nine (50%), serum amyloid protein A in 13 (72%), and hypersensitive C-reactive protein in ten (56%). Eight (7%) of 119 Ixodes persulcatus ticks removed from participants were positive for YEZV. Three YEZV strains were isolated from the sera of patients. Ten viral genomes were obtained from five patients, a blood-sucking I persulcatus removed from a participant, and four host-questing tick samples collected in the areas where patients were identified or in the adjacent region. Phylogenetic analyses revealed that YEZVs in either patients or ticks were divided into two clades, each with distinct mutations.

    Interpretation
    Awareness of YEZV infection is important and clinicians should consider the virus when diagnosing patients with suitable symptoms.

    Abstract
    In March 2024, the Pan American Health Organization (PAHO) issued an alert in response to a rapid increase in Oropouche fever cases across South America. Brazil has been particularly affected, reporting a novel reassortant lineage of the Oropouche virus (OROV) and expansion to previously non-endemic areas beyond the Amazon Basin. Utilising phylogeographic approaches, we reveal a multi-scale expansion process with both short and long-distance dispersal events, and diffusion velocities in line with human-mediated jumps. We identify forest cover, banana and cocoa cultivation, temperature, and human population density as key environmental factors associated with OROV range expansion. Using ecological niche modelling, we show that OROV circulated in areas of enhanced ecological suitability immediately preceding its explosive epidemic expansion in the Amazon. This likely resulted from the virus being introduced into simultaneously densely populated and environmentally favourable regions in the Amazon, such as Manaus, leading to an amplified epidemic and spread beyond the Amazon. Our study provides valuable insights into the dispersal and ecological dynamics of OROV, highlighting the role of human mobility in colonisation of new areas, and raising concern over high viral suitability along the Brazilian coast.
